Historically, water birth originated in early 19th-century France. Recognized internationally as a "return to nature" birthing method, it is noted for shorter labor duration, reduced pain, and smaller perineal tears.
The principle behind water birth stems from the fact that babies develop in the mother's amniotic fluid. Immediate contact with water at birth helps stabilize both the infant's and mother's emotions. Scientific "water birth" requires specialized bathtubs and water maintained at a constant 37°C (98.6°F) with continuous circulation. The water formulation is also unique, designed to mimic the properties of amniotic fluid.
However, not all expectant mothers can opt for water birth. Experts explain that physical eligibility is required. Based on typical conditions for Chinese mothers, the fetus should ideally weigh around 3 kilograms without being too large, present in a normal position, and the mother must have no pregnancy complications to qualify for water birth.
Why is water birth so appealing?
First, water birth facilitates relaxation.
Warm water calms both body and mind, reducing the secretion of stress hormones that can elevate blood pressure and prolong labor. It relaxes muscles, conserves energy reserves for uterine contractions, and eases tension in pelvic floor and leg muscles, facilitating fetal passage through the birth canal.
Second, water birth provides natural pain relief.
Excessive tension and fear during labor can intensify pain. In water birth, signals from skin stimulation transmitted via fast nerve fibers can block or reduce pain signals reaching the brain, diminishing perceived discomfort.
Third, water birth shortens labor duration.
In water, relaxed muscles facilitate cervical dilation and accelerate labor progression. Studies show water birth significantly reduces the first stage of labor compared to bed delivery, saving an average of 85 minutes.
Fourth, water birth reduces birth canal tears.
To prevent perineal tearing during delivery, women with a short perineal body often require an episiotomy. Mu Shixia notes that currently, 70% of primiparas undergo this procedure. Water birth enhances perineal elasticity and relaxation while reducing fetal pressure on the perineum, thereby lowering the risk of birth canal tears and avoiding the need for episiotomy.
Fifth, it facilitates bonding with the baby.
In traditional delivery, the baby is immediately exposed to cold air and bright light upon birth—a sudden transition that often causes fear and distress. Having spent the first nine months developing in the amniotic fluid within the mother's womb, babies possess a natural affinity for water. Water birth provides a sense of security for the newborn.
Who is suitable for water birth?
Water birth is not suitable for everyone and has specific eligibility criteria:
First, the optimal age range for water birth is 20-30 years old. Younger women may lack sufficient psychological preparation, while those over 30 are considered "advanced maternal age." Due to physiological factors, cesarean delivery is generally recommended for this group.
Second, women with medical conditions or a history of miscarriage should opt for a more secure delivery method. Medical conditions can often trigger complications, causing unnecessary harm.
Finally, women carrying a macrosomic fetus (over 7 jin) are not suitable for water birth.
Additionally, the following groups are not suitable for water birth: those who are significantly older or younger; those with medical conditions;those unaware of prenatal conditions; cases of macrosomia; women with a history of miscarriage; pregnancies involving twins or multiples, premature labor, or rupture of membranes lasting over 24 hours.
What to Consider for Water Birth
During water birth, the husband may be present to effectively alleviate the mother's anxiety. Beyond medical assistance and encouragement, the mother also benefits from the companionship and comfort of loved ones.Most mothers giving birth in water can have family members present throughout. Their emotional support plays a significant role in facilitating a smooth delivery. Meanwhile, the father-to-be can film the birth from beside the birthing tub, capturing the baby's arrival while providing comfort to the mother and helping her navigate the labor process. Another distinct advantage of water birth is that mothers can move freely in the water, improving their mental state and facilitating delivery.
Water birth can also effectively shorten labor duration, reduce pain, and protect the perineum. Immersion in water decreases secretion of renin, aldosterone, angiotensin, and vasopressin while increasing natriuretic hormone and dopamine. This leads to increased urine output and lowered blood pressure. Warm water dilates blood vessels, accelerates metabolism and blood circulation, and enhances muscle relaxation.
Buoyancy in water allows mothers to freely adopt comfortable positions, promoting muscle relaxation and reduced anxiety. This increases oxytocin secretion and strengthens uterine contractions, accelerating cervical dilation and shortening labor. The upright position commonly used during water birth protects the perineum by minimizing pressure differentials between the vaginal cavity and external environment, reducing the risk of perineal tears.Postpartum recovery is notably swift for water birth mothers, with some able to leave the hospital the day after delivery.
Childbirth is a profound life event for women, marking the true beginning of motherhood. However, improper management during labor can impact a mother's lifelong physical and mental health. Beyond traditional methods, expectant mothers with suitable conditions may consider water birth, as its benefits significantly outweigh those of conventional delivery.
