Nasal Tip Reshaping: Perfecting the Details!

Many individuals perceive their noses as unattractive due to a deformed nasal tip, such as an overly large or low nasal tip. These conditions can be corrected through nasal tip reshaping surgery. There are various types of nasal tip deformity correction procedures. Below, we introduce common surgical techniques specifically targeting symptoms of an overly large or low nasal tip.This occurs due to thick skin, abundant subcutaneous tissue, and a thick cartilage framework with outward bulging. Correction surgery for an oversized nasal tip is termed rhinoplasty. Treating this type of nasal tip is highly challenging. The procedure employs either a marginal incision or an intercartilaginous incision. The alar cartilage and lateral nasal cartilage are fully exposed and mobilized, with uniform excision of fibrofatty tissue from all relevant areas.Remove 1/2 to 2/3 of the lateral aspect of the alar cartilage head, then rotate the lateral crus medially and cephalad. This action will cause the alar base to retract. Make intermittent incisions along the dome of the alar cartilage without severing it. If reinforcing the nostrils or horizontal nostrils fails to convert them to a vertical orientation, perform a wedge resection at the alar base.If the nasal tip lacks height, the excised cartilage can be placed over the dome surface to elevate the tip. Low Nasal Tip Cartilage Grafting Method. Make a nasal margin incision inside the nostril or an intranasal incision to expose the alar cartilage and lateral nasal cartilage. Depending on the situation, resect the cephalic portion of the alar cartilage or the caudal portion of the lateral nasal cartilage.The harvested cartilage is sculpted and sutured to the apex of the nasal dome to elevate the tip. Alternatively, conchal cartilage from the ear can be harvested for grafting.
Bilateral nasal alar cartilage medial crus approximation.Make bilateral lateral columellar incisions to expose the medial crura of the alar cartilages and the nasal dome. Resect the soft tissue between the two medial crura, then perform a mattress suture near their upper and lower ends to bring the medial crura closer together. This technique can elevate the nasal tip by 2-3mm.
Wedge resection of the alar base.When the nasal tip shape is generally good but slightly low, a wedge-shaped full-thickness tissue segment is excised from both alar bases. This narrows the nostrils and elevates the dome area, thereby lifting the nasal tip.
