Neurodermatitis shares similarities with TCM conditions like "psoriasis" and "cervical eczema." It arises from wind-dampness stagnating in the skin and impaired meridian qi flow. This dermatological disorder commonly affects the neck, limbs, and lumbosacral region, presenting primarily as symmetrical skin roughness, thickening, and intense itching.Neurodermatitis, also known as chronic simple lichenification, is a chronic skin condition characterized by paroxysmal itching and lichenified skin. It is a common, recurrent dermatological disorder primarily affecting young adults and adults, with children generally not being affected. It often occurs more frequently in summer or may show no distinct seasonal pattern. Below is an introduction to the symptoms of neurodermatitis.
Symptoms of Neurodermatitis:
1. Rashes commonly appear on the neck, extensor surfaces of the limbs, lumbar region, popliteal fossae, and perineum;
2. Clinically classified into localized and disseminated types, with the former being more prevalent;
3.Initially, localized itching occurs. After repeated scratching or rubbing, small, round or polygonal flat papules appear, ranging in size from millet to mung bean. These papules are skin-colored, pale red, or light brown with a slight sheen. Over time, the number of lesions increases and they merge into patches, forming typical lichenoid lesions. These lesions vary in size and shape, with scattered flat papules often present around the periphery.
4. Severe itching is experienced. The course is chronic, with potential for recurrent flare-ups or persistent, unresolved symptoms.
What are the treatments for neurodermatitis?
The clinical presentation of neurodermatitis typically begins with dry, rough skin, possibly accompanied by papules, scaling, and intense itching. Over time, the skin thickens, skin furrows deepen, pigmentation occurs, and lichenification develops.Some rashes are confined to specific areas, termed localized neurodermatitis; others spread across limbs and trunk, known as generalized neurodermatitis.
Traditional Chinese Medicine attributes neurodermatitis to emotional distress, repeated mechanical friction, or dietary/lifestyle imbalances. These factors lead to liver meridian heat accumulation, blood deficiency with wind-dryness, and qi stagnation with blood stasis.
Modern medicine views this condition as a neurofunctional dermatosis. While the exact etiology remains unclear, it is generally associated with cerebral cortex dysfunction and local physical stimuli.
The skin lesions of neurodermatitis are characterized by dryness, roughness, thickening, scaling, and intense itching. Its onset is most closely associated with wind, dryness, blood deficiency, and blood stasis. Consequently, treatment principles commonly focus on dispelling wind, moistening dryness, nourishing blood, and promoting blood circulation to resolve stasis.Additionally, sedative and heart-nourishing herbs are frequently employed, such as pearl oyster shell, hematite, Albizia bark, Poria cocos, jujube seed, dragon bone, and oyster shell. For cases complicated by damp-heat, dampness-resolving and heat-clearing agents should be added, including smilax root, raw rehmannia, white fresh bark, and sophora root.
Numerous external therapies exist for neurodermatitis, including external washing, topical application, external rubbing, blistering therapy, scarification therapy, medicinal moxibustion, acupuncture, acupoint injection therapy, plum blossom needle therapy, and moxibustion. Each demonstrates varying efficacy, allowing clinicians to select one or multiple approaches based on individual cases.For instance, in the early stages with mild symptoms, external washing, topical application, plum blossom needle therapy, and medicinal fumigation may suffice. For prolonged, severe, or stubborn cases, therapies like scarification and acupoint injection can be added. Since many external treatments for neurodermatitis involve irritating or toxic substances, it is crucial to understand the indications and avoid excessive use to prevent adverse reactions.
