Two Types of Rhinoplasty for Correcting a Bulbous Nose Tip

A large nasal tip can make the face appear bulky, while a smaller tip creates a slimming effect. Beauty-conscious individuals may consider nasal tip reduction surgery, which includes two approaches: high nasal tip correction and low nasal tip correction. The choice depends on the individual's nasal structure.
Two Methods of Nasal Tip Reduction
I. Hyper-Rhinoplasty
This technique is suitable for overly prominent nasal tips characterized by a steep, straight elevation from the nasal bridge to the tip. A significant curvature in this elevation is termed hyper-rhinoplasty. Post-surgery results are generally favorable.An incision is made between the cartilages. II. Low Nasal Tip Correction
1. Cartilage Grafting Method
For nasal tip reduction, an incision is made along the nasal margin inside the nostril or within the cartilage. The alar cartilage and lateral nasal cartilage are exposed. Depending on the situation, the cephalic portion of the alar cartilage or the caudal portion of the lateral nasal cartilage is excised.The harvested cartilage is sculpted and sutured to the apex of the nasal dome to elevate the nasal tip. Conchal cartilage from the ear may also be harvested for grafting.
2.Bilateral Nasal Alar Cartilage Medial Foot Adduction Technique
Make bilateral columellar lateral rhinoplasty incisions to expose the medial feet of the alar cartilages and the nasal dome. Resect soft tissue between the two medial feet, then perform mattress sutures at their proximal and distal ends to adduct the medial feet.This technique elevates the nasal tip by 2-3mm.
3. Wedge Resection of Nasal Alar Base
For cases with a well-shaped but slightly low nasal tip, perform a wedge-shaped full-thickness resection at the bilateral alar bases. This narrows the nostrils, elevates the dome, and consequently raises the nasal tip.

Rhinoplasty for nasal tip reduction employs four incision techniques: two within the nostrils and two along the nostril margins. Different incision approaches yield distinct outcomes, such as narrowing a bulbous tip or elevating a low-set tip.
