Congenital defects refer to developmental disorders present before birth, including structural abnormalities, metabolic disorders, congenital malformations, and intellectual disabilities. Since these developmental issues occur prenatally, they are termed congenital defects and typically fall into four categories:
1. Congenital Malformations
These include visible external abnormalities such as cleft lip or syndactyly;and internal organ/tissue malformations like hydronephrosis or congenital heart disease.
3. Intellectual disability
Includes congenital intellectual impairment caused by various chromosomal abnormalities, such as Down syndrome, lead poisoning, and congenital rubella syndrome.
4. Developmental delay
Although influenced by genetic factors, it is more often caused by environmental factors such as malnutrition, alcohol abuse, smoking, and intrauterine infections.Most cases present with physical deformities at birth, accompanied by impaired intellectual development.
Five Major Causes of Congenital Defects
Cause 1: Negative Emotions
Potential defects: Cleft lip, ADHD
Weeks 7-10 of pregnancy mark the critical period for fetal lip and palate development.If the mother experiences excessive tension, depression, fear, anger, or prolonged anxiety, hormonal imbalances may occur in the hormones transmitted to the fetus. This can severely impact fetal development, potentially leading to deformities like cleft lip or cleft palate, or even miscarriage.
In late pregnancy, maternal emotional distress can disrupt the balance between the fetus's brain and internal organs, affecting personality development and leading to "behavioral disorders" such as ADHD, picky eating, excessive crying, and frequent temper tantrums.
Preventive Measures:
Maintain emotional stability from preconception through delivery.Regularly remind yourself with wise sayings to maintain a positive mindset, striving to avoid idle thoughts, unpleasant sounds, and unsightly sights. There is no need to constantly worry about fetal development, labor pains, difficult delivery, postpartum lactation issues, or body changes. Keep your mind calm and composed. Whenever feeling angry or irritable, remind yourself that such emotions are detrimental to the fetus and help adjust your mood.
Reason 2: Folate Deficiency
Potential Congenital Defects: Anencephaly, Spina Bifida, etc.
Folate is a water-soluble vitamin that cannot be synthesized by the body and must be obtained from folate-rich foods.Insufficient intake before or during pregnancy can lead to megaloblastic anemia in the mother. During the critical period of 3-8 weeks after conception, this deficiency may further induce neural tube defects in the embryo. If the anterior end of the neural tube fails to close properly, anencephaly may result; if the posterior end fails to close, spina bifida may occur, leading to severe complications after birth.
Preventive Measures:
Data indicates that due to improper cooking habits and insufficient dietary intake of folate-rich foods, folate deficiency is prevalent among women of childbearing age in China.Currently, the most effective methods to prevent folate deficiency include consuming folate-rich foods such as animal liver, spinach, lettuce, watermelon, hawthorn, bayberry, and cherries, as well as correcting cooking habits—specifically, avoiding excessive cooking times and temperatures when preparing vegetables.More importantly, begin taking folic acid supplements such as Siliang or Materna three months before conception and continue until three months after pregnancy.
Cause 3: Viral Infections
Potential congenital defects: Microcephaly, microphthalmia, congenital cataracts, glaucoma, deafness, hydrocephalus, congenital heart disease, and malformed fingers/toes.Cytomegalovirus (CMV), Herpes Simplex Virus (HSV), and other viral infections, collectively known as TORCH. This group of pathogens causes fetal infections, leading to birth defects or developmental abnormalities. Generally, when pregnant women contract TORCH pathogens, most cases present as asymptomatic infections without obvious symptoms, yet they can severely impact the fetus.In recent years, perinatal TORCH screening has been implemented in Western developed countries and some domestic hospitals. This involves testing blood or cervical secretions from pregnant women, with immediate intervention upon detecting positive results. Preventive Measures Against Defects: For rubella virus, prevention is currently the primary approach. Women can receive the rubella live attenuated vaccine before pregnancy, but should avoid conception for three months post-vaccination.If infection is suspected during early pregnancy, undergo an IgM antibody test at a hospital. If acute rubella is confirmed, termination of pregnancy should be considered.
Avoid contact with pets like cats when preparing for pregnancy. If toxoplasmosis infection is confirmed through testing, initiate treatment immediately and wait until serological antibodies turn negative before considering pregnancy. If infected with toxoplasmosis during early pregnancy, undergo abortion to prevent birth defects.
Avoid contact with chickenpox patients before and during pregnancy. Since most women have immunity to the varicella-zoster virus, early pregnancy infection carries low fetal risk and may proceed. If infected near delivery, administer varicella zoster immune globulin.
Women with primary herpes simplex virus infection should wait until symptoms resolve before considering pregnancy.Once pregnant, avoid contact with individuals infected with herpes simplex virus.
Congenital abnormalities caused by cytomegalovirus infection often manifest months or even years after birth, typically as intellectual disability. Before conception or during pregnancy, avoid crowded places, maintain good hygiene, and prevent contact with infected individuals. If infection is confirmed, consider pregnancy termination to prevent birth of a child with congenital defects.
Cause 4: Inappropriate Medication Use
Potential congenital defects: Congenital deafness, kidney damage, missing fingers or limbs, cleft lip, cleft palate, genital abnormalities, etc.
Fetal exposure to certain medications—whether intentional or unintentional—before or during conception can cause harm.The most overlooked period is between days 15-56 of pregnancy (equivalent to 30-72 days after the last menstrual period), which coincides with the highly sensitive period for teratogenic effects. While most medications are safe for the fetus, certain drugs used during this critical window can cause severe harm to fetal growth and development.
Preventive Measures:
Both partners should exercise caution with medication use when planning pregnancy or during the first trimester. Women of childbearing potential should consider menstrual timing when taking any medication. Pregnant individuals experiencing discomfort should seek prompt medical attention, accurately informing the physician of their conception date to ensure selection of fetus-safe medications. Always strictly follow medical instructions when taking drugs; never self-medicate.
Cause 5: Radiation Exposure
Potential Congenital Defects: Microcephaly, mental retardation, microphthalmia, hydrocephalus, etc.
Early pregnancy, particularly days 15-56, coincides with critical organ differentiation and formation. X-ray exposure during this period carries a high risk of causing fetal abnormalities due to its potent teratogenic effects.
Preventive Measures:
Avoid abdominal X-rays entirely during the first trimester. Postpone chest X-rays until after 28 weeks of pregnancy. Pelvic X-rays or chest radiographs should be scheduled after 36 weeks. If exposed to X-rays or radioactive isotope treatments/exams before knowing you were pregnant, consult your doctor about continuing or terminating the pregnancy.If employed in radiation-related work, it is advisable to temporarily transfer to non-radiation duties during pregnancy.
Sensitive Period for Congenital Defects
Medical research indicates that the period when embryonic organs undergo rapid differentiation—specifically 15–56 days post-conception, or 30–72 days from the last menstrual period (the first trimester)—is highly sensitive to environmental teratogenic factors.During this time, exposure to various external stimuli can easily cause malformations. For example: - The brain's highly sensitive period is 15-27 days post-conception - The heart's sensitive period is 20-30 days post-conception - Limbs develop during 24-36 days post-conception - Reproductive organs form between 38-60 days post-conception Since some organs develop simultaneously, a single external stimulus may sometimes cause malformations in multiple organs.
