Qing Tianhe Water: Right or Left Hand?
Qing Tianhe Water is typically applied to the patient's left hand. Its primary function is to clear internal heat and dispel excess fire. Effects are generally more pronounced when performed on the left side of the body. Combining both sides is also feasible, as acupoints are symmetrically distributed. For instance, begin on the left upper limb and proceed to the right upper limb during subsequent sessions.
Illustration of the "Clearing the Heavenly River" Technique
The "Clearing the Heavenly River" point is located along the central line of the forearm, from the wrist crease to the midpoint of the elbow crease. The practitioner or caregiver typically grasps the child's wrist from the back with the left hand, then uses the index and middle fingers of the right hand to push from the wrist crease toward the midpoint of the elbow crease. This must be done in one direction only, not back and forth. If the palm side of the forearm is simultaneously tapped while pushing, this is called "Beating the Horse Across the Heavenly River."
Distinguishing "Clearing the Heavenly River" from "Drawing the Heavenly River"
These techniques differ in both method and therapeutic effect.
Clearing the Heavenly River involves pushing from the wrist crease to the elbow crease. It has the effect of dispersing wind-heat, reducing fever, and expelling pathogens. It is used for fever without sweating and superficial syndromes. Drawing the Heavenly River involves pushing downward from the elbow crease to the Laogong point. It has the effect of nourishing yin and reducing fire. It is used for conditions such as internal heat syndrome, night sweats, and hot hands and feet.>Precautions for the Heavenly River Water Technique
Avoid applying this technique to children with fever indiscriminately, as it drains yang energy. Fever typically signifies the body's vital energy expelling pathogens—the heat itself represents this vital force. Assisting the body's fever helps expel pathogens; draining energy weakens this vital force. Forcing rapid cooling undermines the body's defense mechanisms. Generally, if a feverish body maintains strong immunity, it can manage the condition independently.
Contraindications for Clearing the Heavenly River to Reduce Fever
This technique clears heat from the heart and liver meridians, proving effective for fevers caused by excessive heart fire or liver stagnation in children. However, it should not be used for colds resulting from exposure to cold. Additionally, avoid this method if the child has skin rashes or open sores on the arms.
