Molar pregnancy has long been one of the common gynecological conditions. Although its causes are numerous, the exact etiology remains unclear, leaving many pregnant women unaware of how to prevent it early. Two Major Symptoms of Malignant Transformation in Molar Pregnancy I. Hemoptysis Coughing up blood.Malignant hydatidiform cells can invade blood vessels and travel through the circulatory system, easily reaching the lungs where they settle, grow, and multiply, destroying surrounding healthy lung tissue. When these malignant cells damage pulmonary blood vessels, hemoptysis occurs.If irregular vaginal bleeding persists for more than 3–4 weeks after the hydatidiform mole is expelled—characterized by intermittent flow, varying intensity, and prolonged spotting—malignancy should be suspected. Additionally, malignant hydatidiform cells can metastasize via the bloodstream to the uterine body, vagina, or vulva. They may infiltrate surrounding tissues, ulcerate spontaneously, or rupture due to accidental pressure, all of which can cause vaginal bleeding.In rare cases, malignant cells may perforate the uterine wall, causing uterine rupture. Blood flowing into the abdominal cavity can lead to coma, shock, and death. Beyond these two points, other symptoms may occur. For instance, if malignant cells metastasize to the brain, patients may experience headaches, dizziness, and vomiting. Metastasis to the gastrointestinal tract can cause symptoms like vomiting blood, bloody stools, or tarry stools.The consequences of malignant transformation in a molar pregnancy are severe, but early diagnosis and treatment offer a favorable prognosis. If any of the above symptoms are detected, prompt medical evaluation is essential. This awareness is particularly crucial for individuals with a history of molar pregnancy.
Complications of Molar Pregnancy:
1. Massive Hemorrhage: If a molar pregnancy is not promptly diagnosed and managed, recurrent bleeding may occur, leading to uterine cavity hematoma and significant blood loss. Heavy bleeding can also occur during spontaneous expulsion.In patients already anemic, this can progress to hemorrhagic shock and even death. Therefore, molar pregnancy must be treated as an emergency. Even brief delays can cause further blood loss and endanger the patient.Patients with hydatidiform mole who experienced spontaneous abortion shortly before admission and are able to tolerate curettage should undergo immediate uterine evacuation. For those with prolonged expulsion or signs of infection, antibiotic prophylaxis should be administered for several days before curettage.
III. Hydatidiform Mole Embolism Hydatidiform moles can metastasize or migrate via the bloodstream to other body parts, most commonly the lungs and vagina, potentially forming local hemorrhagic foci.Small emboli or cases not thoroughly examined may resolve spontaneously. Yu Peiliang et al. reported a case where oxytocin-induced labor triggered extensive pulmonary metastasis of a hydatidiform mole, leading to pulmonary arteriolar spasm syndrome and death from pulmonary edema and heart failure. Unlike malignant tumor metastasis, hydatidiform mole emboli can sometimes be eliminated by the body's own immune suppression, as documented by Lin Qiaozhi and Su Yingkuan.Currently, chemotherapy remains the preferred treatment upon detection.
IV. Malignant transformation into invasive hydatidiform mole or choriocarcinoma. The malignant transformation rate is approximately 10%–20%.
V. Torsion of the ovarian luteinized cystocele typically occurs after hydatidiform mole expulsion. Immediate surgical resection of the twisted uterine adnexa is indicated upon torsion onset.
