There are three main types of lower eyelid bag surgery: artificial dermal lower eyelid bag augmentation, autologous dermal lower eyelid bag augmentation, and autologous fat grafting lower eyelid bag augmentation.
Artificial Dermal Lower Eyelid Augmentation
This procedure begins by retracting the lower eyelid and administering local anesthesia via injection into the conjunctival surface. A 2-3mm transverse incision is then made along the eyelid margin, followed by layer-by-layer dissection.Finally, synthetic dermal tissue of an appropriate shape is transplanted into the incision site, followed by hemostatic suturing.
Autologous Dermal Lower Eyelid Augmentation
This procedure also begins with anesthesia. An appropriate amount of dermal tissue is harvested from a discreet area of the patient's body, shaped, and prepared for use. A 2-3mm transverse conjunctival incision is then made along the lower eyelid margin, followed by layer-by-layer dissection. The prepared dermal graft is subsequently transplanted.
Autologous Fat Particle Injection for Lower Eyelid Augmentation
This procedure begins with liposuction to harvest fat. Purification techniques are then used to isolate viable fat particles for injection. Following local infiltration anesthesia, the purified fat particles are injected into the subcutaneous fat layer of the orbital rim. Postoperative massage for shaping and appropriate compression bandaging may be applied.
Due to the relatively low survival rate of autologous fat grafts, a second injection can be performed after 3 months if the results are unsatisfactory.
