The procedure and steps for facial liposuction are as follows: 1. Preoperatively, mark the areas of localized fat accumulation with gentian violet. Perform repeated palpation to assess the thickness of the local fat tissue, emphasizing thicker areas with gentian violet.
2. Administer local tumescent anesthesia using a 0.1%–0.5% lidocaine saline solution. Through the incision, evenly inject the anesthetic into the subcutaneous layer of the liposuction area until the skin feels firm and distended.
3. Select puncture sites at the following locations: below the earlobe, the vermilion border of the lips, and the midline of the chin. Make a 2mm incision. Use a 2mm-diameter long cannula with a rounded tip and lateral ports. Attach the cannula to a 50ml disposable syringe. Insert through the incision into the subcutaneous fat layer.First advance the needle to its farthest point and create several subcutaneous tunnels without applying negative pressure. Then pull the plunger to establish suction. Perform repeated sawing-like radial suction movements, applying more pressure in areas with greater fat accumulation while ensuring uniform suction until the desired result is achieved. However, leave a subcutaneous fat layer approximately 0.8mm to 10mm thick.
4. Typical fat removal volumes are: cheeks 10–30 ml, cheekbones 5–30 ml, and jawline 20–35 ml.
5. At the end of the procedure, gently squeeze the skin over the liposuction area to expel accumulated fluid. Close the incision with a single suture and apply a compressive gauze dressing.
7. Facial liposuction demands high technical proficiency. Surgeons must be thoroughly familiar with local anatomy, pay attention to the layers and methods of suction, and avoid suctioning outside the subcutaneous fat layer. Apply uniform force within the same plane. Avoid excessive repetition, excessive force, or overly rapid suction frequency within the same tunnel to prevent excessive thinning of the skin at any single point.

Postoperative Care for Facial Liposuction:
Apply cold compresses with compression immediately after surgery to prevent hematoma or seroma formation. Maintain compression bandaging with elastic dressings for 48 hours.Infection is uncommon after facial liposuction. If infection occurs, promptly change dressings and, if necessary, irrigate with antibiotic solution or administer systemic antibiotics. Avoid strenuous facial movements and hard foods for 2 weeks post-op. Resume normal activities after 3–4 weeks.
