Using the lateral one-third of the nose as the pedicle, an incision is made in the temporal two-thirds of the eyebrow area. This brings the temporal one-third and nasal one-third into proximity, temporarily forming a fold in the middle one-third.After 3 weeks, use the temporal third as a pedicle to flatten the fold, transpose it medially toward the nasal side, and suture. If both sides have less than one-third nasal-side defects, bilateral procedures may be performed simultaneously.
IV. Contralateral Eyebrow Flap Transplantation.Performed under local anesthesia. Make an arc-shaped incision at the affected side corresponding to the brow ridge. Subcutaneously dissect to create a wound bed matching the normal eyebrow width. Divide the healthy eyebrow into upper and lower sections, make transverse incisions, then create parallel incisions above the eyebrow to form a flap with the medial end as the pedicle, matching the width of the affected wound bed. Rotate and transplant the flap to the affected wound bed.
V. Scalp Transplantation Methods:
1. Retroauricular Scalp Flap Method: Suitable for unilateral or bilateral eyebrow defects, or sparse eyebrows. Performed under local anesthesia. For unilateral defects, design the affected side based on the healthy eyebrow. For bilateral defects, design based on an ideal eyebrow shape.Make a local incision and dissect the wound bed, ensuring it is slightly wider than the designed eyebrow shape to prevent narrowing due to insufficient hair growth from damaged follicles at the scalp harvest margin. Harvest a graft from behind the affected ear and transplant it onto the pre-dissected wound bed on the affected side. Directly suture the donor site.Island Flap Technique: Following the course of the superficial temporal artery, an island scalp flap is designed according to the extent of eyebrow deficiency and transplanted into the affected area. Suitable for male patients with thick eyebrows.
3. Temporal Lateral Flap Technique: A narrow, elongated flap is designed in the temporal region ipsilateral to the eyebrow defect. The flap undergoes delayed transfer in one stage.
4. Ring Drill Cluster Grafting: Suitable for smaller defects. The technique resembles rice transplanting. Use a ring drill to create small patterned defects in the affected eyebrow area. Simultaneously harvest grafts from denser scalp regions using a drill, then transplant them onto the pre-prepared patterned defects with light pressure and bandaging.
