Dietary considerations for urinary stones

Urinary tract stones are a common condition affecting the urinary system in adults. Their occurrence is closely linked to dietary habits. Therefore, preventing the formation and progression of urinary stones depends not only on maintaining adequate hydration and regular exercise but also significantly on dietary choices. The following guidelines outline recommended and discouraged practices. Recommended: Primarily focus on three aspects: dietary fiber intake, calcium-rich foods, and vitamin-rich foods.
1. Prioritize dietary fiber intake: Studies indicate a negative correlation between dietary fiber consumption and urinary stone incidence. Increasing fiber intake reduces urinary calcium oxalate and uric acid levels. Whole grains, tubers, and fresh vegetables are rich in fiber. Combining coarse and refined grains with ample fresh vegetables effectively boosts dietary fiber intake.
2. Increase calcium-rich food intake: Low calcium levels can cause bone resorption to exceed bone formation, elevating calcium levels in both blood and urine. This also increases oxalate absorption and urinary excretion, leading to oxalate-calcium precipitation and promoting stone formation. Therefore, dietary calcium intake should be increased through foods like milk, fish, shrimp, and seaweed.Note: Avoid using calcium supplements to prevent urinary stones, as excessive calcium intake may actually increase the risk of stone formation.
3. Increase intake of vitamin-rich foods: Vitamin B6 reduces the formation of calcium oxalate crystals in urine, while vitamin K lowers urinary oxalate levels. Therefore, dietary intake of these two vitamins should be increased. Green vegetables like alfalfa, cabbage, and citrus fruits are rich in both vitamins; consuming them appropriately can help reduce the incidence of urinary stones.
Avoid: Primarily involves restricting foods high in oxalate, animal protein, and salt.
1. Limit intake of high-oxalate foods: Most urinary oxalate originates from bodily metabolism, with a smaller portion from food. Nevertheless, reducing intestinal absorption of oxalate is crucial. Avoid foods rich in oxalate, such as chocolate and coffee.
2. Control animal protein intake: Excessive animal protein increases urinary excretion of oxalate and calcium, reduces citrate excretion, lowers urine pH, and elevates stone formation risk. Therefore, limit excessive animal protein in the diet.
3.Avoid high-salt foods: High-salt foods can increase urinary calcium while reducing citrate excretion, thereby raising the incidence of urinary stones. Therefore, use less salt when cooking daily meals, and limit consumption of pickled vegetables, cured meats, and fried foods to maintain a light diet. Simultaneously, ensure adequate water intake to prevent the occurrence and recurrence of urinary stones.Drink plenty of water. Aim for daily fluid intake of 2.5–3.0 liters. Limit caffeine, black tea, grape juice, and cola. Opt for orange juice, tart fruit juices, and lemon water.
2. Patients should adjust their diet based on stone composition analysis. Those with calcium oxalate stones, especially those with hyperoxaluria, should avoid foods like spinach, almonds, peanuts, celery, and black tea.Spinach contains the highest oxalate levels. Patients with recurrent calcium oxalate stones should avoid high doses of vitamin C. Those with uric acid stones should limit high-purine foods (such as organ meats) and alkalinize urine. Avoid excessive vitamin D intake, reduce protein consumption (including meat, fish, and poultry), and minimize salt intake by limiting high-sodium foods.Potassium citrate, as a monotherapy, has been clinically applied for over 20 years to treat distal renal tubular acidosis, hypocitraturic calcium oxalate stones, and uric acid stones with or without calcium-containing stones. It demonstrates preventive and therapeutic effects against various types of urinary tract stones.
