Generally speaking, breast augmentation materials fall into two categories: implant-based augmentation and autologous fat transfer augmentation. The most common direction of implant displacement is upward and outward. Currently, the vast majority of breast augmentation procedures in China place implants beneath the pectoralis major muscle. When creating the pocket, the inner-lower region is relatively tough due to the insertion points of the pectoralis major and rectus abdominis muscles, while the outer-lower ligaments are also quite thick and resilient.Dissection in these areas is challenging, particularly when using an axillary incision. If the lower dissection margin is insufficiently wide while the outer-superior margin is excessively broad, combined with the contraction of the pectoralis major muscle, the implant may gradually be pushed outward and upward, leading to displacement.
Another type of implant displacement is downward migration, which occurs due to excessive dissection at the lower attachment point of the pectoralis major muscle or the lower pole of the breast during augmentation surgery.In subglandular augmentation, displacement often occurs when excessive separation occurs between the skin/subcutaneous tissue and the underlying fascia. Historically, high-concentration steroid injections were sometimes administered into the dissection cavity to prevent capsular contracture. However, when patients stand, the solution migrates toward the lower pole, causing subcutaneous tissue thinning and atrophy, which can also lead to this complication.
