Due to its subtle early symptoms, COPD is often called the "silent killer." As the world's fourth leading cause of death, COPD has a global prevalence rate of 11.7%, surpassed only by cerebrovascular disease, heart disease, and acute pulmonary infections. November 18th is World COPD Day, established by the World Health Organization.Epidemiological surveys in China indicate a prevalence rate of 13.7% among those under 40 and 15.5% among those over 50, with a noticeable trend toward younger onset in recent years. Approximately 960,000 people die from COPD annually in China. Winter is the peak season for COPD exacerbations, particularly for patients with pre-existing chronic airway inflammation. Cold air can irritate the airways, triggering flare-ups or acute worsening, making protective measures especially crucial.
Difficulty breathing is a classic symptom. Chronic obstructive pulmonary disease (COPD) is a common chronic lung condition characterized by airflow obstruction. Persistent respiratory symptoms and restricted airflow are hallmarks of COPD. It typically results from long-term exposure to toxic gases and harmful particles that damage the airways and alveoli.Consequently, smokers and certain occupational groups—such as teachers, miners, and chemical industry workers—are considered high-risk populations. Additionally, burning non-clean fuels like firewood and exposure to cooking fumes can produce toxic gases and particles. Prolonged exposure to such environments also elevates the risk of developing COPD.
Because early COPD symptoms are often subtle, most patients only recognize the condition once severe symptoms have developed.Early clinical manifestations include coughing and phlegm production, but these are often dismissed as ordinary coughs. As COPD progresses to reduced physical endurance, symptoms are frequently attributed to aging or lack of exercise. By the time patients experience shortness of breath and difficulty walking, lung function impairment is severe, and optimal treatment windows are missed.
"In fact, COPD symptoms follow discernible patterns," cautions Dr. Dai Jiechen, attending physician in the Emergency Department at Guangdong Provincial Hospital's Ersha Island Branch. First, COPD coughs recur frequently, with thicker sputum production during acute infections—particularly noticeable in the morning. Second, COPD diminishes physical endurance, manifesting as reduced daily functioning and work capacity compared to peers.Third, during acute exacerbations, patients may exhibit open-mouth breathing with elevated shoulders. Fourth, symptoms like palpitations and chest tightness may occur. Fifth, patients may experience reduced appetite and weight loss. In older patients, COPD can also contribute to osteoporosis.
How can we stop COPD from remaining "silent"? Dr. Dai Jiechen outlines five key self-assessment criteria: 1. Age over 40;2. Recurrent coughing with phlegm production; 3. Reduced exercise tolerance and poorer physical stamina compared to peers; 4. Difficulty breathing; 5. Status as a smoker or high-risk exposure individual. Meeting any three of these criteria warrants prompt medical evaluation with pulmonary function testing. Early detection, diagnosis, and treatment can significantly prevent or mitigate COPD-related harm.
