Seafood is delicious, and many people love to eat it. When handling and cooking seafood at home, many people may not take prick injuries seriously. Experts warn: Don't assume this is a minor issue. If a small wound is not treated promptly, it could lead to accidental infection with the invisible ocean killer—Vibrio vulnificus. Thumb Injured by Crab, Swelling, Pain, Persistent High Fever
Mr. Cai (pseudonym), 67, suffers from diabetes and gout but remains relatively healthy, occasionally indulging in seafood. Recently, while cleaning crabs at home, he accidentally pinched his right thumb. The wound was small, so he didn't think much of it.
That evening, Mr. Cai experienced swelling, pain, and fever in his thumb.The next day, the tip of his thumb darkened, the pain intensified, and his temperature reached 39°C (108.6°F). His family took him to the hospital, where antibiotics failed to improve his condition. He developed symptoms including fatigue, chest tightness, palpitations, and cold sweats, while his finger grew increasingly swollen and painful. They then rushed him to the emergency department at Panyu Campus of Guangzhou Medical University Second Affiliated Hospital. Due to his persistent high fever, Mr. Cai was directed to the fever clinic.The attending physician recognized signs of shock and critical condition, immediately ordering blood tests, nucleic acid testing, and consulting orthopedics. Given the blackening of his finger, doctors suspected a possible infection with Vibrio vulnificus!
Rapid Progression Demanded Amputation to Save His Life
After admission, doctors administered anti-infective therapy, fluid replacement, and anti-shock treatment.By 5 p.m. that day, unbearable swelling and pain in his thumb, along with blackening of the fingertip and redness/swelling in his right hand, suggested acute necrotizing fasciitis.
Doctors immediately performed a distal thumb decompression and debridement under local anesthesia. Post-surgery, his pain eased, but by 7 PM he became restless with accelerated heart rate, indicating septic shock. By 10 PM, Mr. Cai exhibited impaired consciousness and worsening agitation, with swelling spreading to his upper right arm. With family consent, he was transferred to the ICU.
In the ICU, physicians continued treatment with antimicrobial therapy, fluid resuscitation, nutritional support, sedation, and analgesia. They also collected exudate from the thumb wound for metagenomic sequencing.The next day, the distal phalanx of Mr. Cai's right thumb turned completely black with no circulation. Tension blisters and subcutaneous ecchymosis appeared on his palm. Doctors immediately performed decompression debridement under local anesthesia on his thumb, thenar eminence, dorsal hand, and forearm.
Postoperatively, swelling in Mr. Cai's hand subsided. However, on the third day, his mental status deteriorated with incoherent speech. The distal phalanx of his right thumb had turned black and shriveled, emitting a foul odor, while his right upper arm and shoulder exhibited marked redness and swelling. With family consent, doctors urgently performed amputation of his right thumb.
That same evening, genetic sequencing results confirmed Mr. Cai's infection was caused by Vibrio vulnificus!
Under the dedicated care and close monitoring of medical staff, Mr. Cai's wounds healed well, and he was discharged successfully.
Beware the Invisible Ocean Killer—Vibrio vulnificus
Vibrio vulnificus is a common marine弧形bacterium found in seawater and on the surfaces of seafood. Experts indicate this bacterium can be detected in shellfish, crabs, mollusks, and fish/shrimp products.
Clinical observations reveal that most confirmed cases occur when wounds on the distal ends of toes or fingers come into contact with seawater or seafood contaminated by Vibrio vulnificus.
Generally, healthy individuals with strong immunity or those frequently exposed to seawater and seafood often experience mild symptoms or no illness after infection. However, high-risk groups—such as those with diabetes, rheumatoid arthritis, chronic kidney failure, liver disease, chronic alcoholism, or taking immunosuppressive drugs—have relatively weakened immune function and disease resistance. Once infected, their skin may rapidly develop symptoms like redness, swelling, heat, and pain.When large quantities of potent toxins released by Vibrio vulnificus and necrotic tissue enter the bloodstream, they can trigger high fever, sepsis, and toxic shock. Without prompt treatment, this can lead to multiple organ failure and even death.
Remember these key points to avoid the "ocean killer"
To prevent Vibrio vulnificus infection, experts advise paying attention to the following key points:
1.Individuals with skin or oral wounds, especially those with compromised immune systems, should avoid swimming in the ocean.
2. Wear gloves when handling seafood to prevent cuts or punctures from marine organisms or sharp objects in seawater. If injured, do not underestimate the risk. Treat the wound promptly according to the following guidelines:
1) Rinse the wound thoroughly to reduce inflammation;
2) Unless bleeding is uncontrollable or the injury is severe, leave the wound fully exposed without bandaging;
3) If the wound shows clear signs of infection such as redness, swelling, heat, or pain, seek medical attention promptly and inform the doctor about exposure to seawater or seafood to prevent misdiagnosis.
3. When handling or cooking seafood at home, use separate utensils for raw and cooked seafood.
4. Consuming undercooked seafood contaminated with Vibrio vulnificus can also lead to infection. Always ensure seafood is thoroughly cooked before eating.
