Sigh! All I can say is, you guys got incredibly lucky! While international standards don't mandate vaccines for every dog bite, our national circumstances differ. Let me explain in detail.
1. Can you use the 10-day observation method after a dog bite? Debunking the myth
Rabies, also known as hydrophobia, has a nearly 100% fatality rate once symptoms appear. It's prevalent in over 150 countries worldwide—found everywhere except Antarctica—but over 95% of fatal infections occur in Asia and Africa.
The disease prevalence rate reaches 42% in East and Southeast Asia, 9% in Africa, 7% in Latin America, and 3% in Eastern Europe. Thailand, Indonesia, Nepal, China, and India have the highest rabies prevalence globally. In China alone, over 2,000 people die from rabies annually on average.
Dogs are the primary source of rabies transmission, accounting for 60% of cases, followed by primates (24%), cats (10%), and bats (2%).
Immediate rabies vaccination after a dog bite is the prevailing consensus domestically. Internationally, however, particularly in developed nations, the "10-day observation method" is also practiced regarding rabies vaccination.
What does this mean? The 10-day observation method refers to the practice where if an animal like a dog or cat remains healthy and shows no rabies symptoms 10 days after biting a person, it indicates the animal's saliva did not contain the rabies virus at the time of the incident, posing minimal risk.
However, please note this approach is not applicable in China. As a high-incidence country for rabies, the fundamental issue lies in the lack of immunization among domestic pets and unregulated dog ownership. Adopting such an intermittent observation approach would likely delay treatment, providing time for the rabies virus to spread.
China spends over 10 billion yuan annually on rabies prevention, yet most resources focus on human vaccination, undermining overall effectiveness. A country or region can eliminate rabies outbreaks by vaccinating over 70% of its dogs (achieving herd immunity). Currently, China's pet management remains in its infancy and is far from meeting this standard, necessitating post-exposure vaccination after animal bites.
Conversely, in countries with low rabies prevalence where the disease is largely eradicated and animals like dogs and cats are routinely vaccinated, the 10-day observation period may be applicable. However, in China, guided by the "Guidelines for Rabies Exposure Prevention and Treatment (2009 Edition)," the safest approach remains administering the vaccine as promptly as possible after a bite.
II. What is the pathogenesis of rabies?
The rabies virus enters through skin or mucous membranes at the bite site, initially multiplying in small quantities. It then binds to acetylcholine receptors on nerve fibers and invades nearby peripheral nerves.
The time from wound entry to peripheral nerve invasion is no less than 3 days.The virus spreads centripetally along the axoplasm of peripheral nerves at a rate of approximately 3 mm per hour (5 cm/day). Upon reaching the dorsal root ganglion, it begins to replicate extensively, then invades the spinal cord and subsequently the brain, affecting the entire central nervous system.
The virus ultimately invades various tissues and organs, with the highest viral loads found in the salivary glands, taste buds of the tongue, and olfactory epithelium.Damage to the vagus, glossopharyngeal, and hypoglossal nuclei causes spasms of respiratory and swallowing muscles. Clinically, patients exhibit hydrophobia, dyspnea, dysphagia, and ultimately death. III. How does the rabies vaccine work? Rabies passive immunotherapy consists of neutralizing antibodies, available in two forms: immune serum and immunoglobulin.During the early vaccination phase, when the body has not yet produced sufficient neutralizing antibodies, passive immunotherapy immediately neutralizes the virus at the wound site, preventing its spread into the nervous system. The half-life of passive immunotherapy is 2–3 weeks, providing the body with time and opportunity to develop active immunity. Combining passive immunotherapy with vaccination maximizes protection against rabies.
IV. How to Properly Treat the Wound?
When conditions permit, after being bitten by a dog, the wound should be treated first, followed by immediate transport to a hospital for vaccination.
After a bite, the primary step is to clean the wound. Rinse it continuously with 20% soap solution and running water for at least 15 minutes.If soap is unavailable in an emergency, rinsing with running water alone is necessary. For deep wounds, syringes or high-pressure irrigation devices may be used to flush deep tissue—though such equipment is rarely available on-site, requiring prompt medical attention.
After rinsing, disinfect the wound with 75% alcohol or 3% povidone-iodine solution.
Crucially, given the current high prevalence of rabies, vaccination must be initiated as soon as possible after any animal bite—delay is unacceptable.
