Dry, thin, and pointed hands with fixed, bent finger joints resembling eagle claws may indicate ulnar or median nerve damage. This can occur in conditions such as syringomyelia, progressive muscular dystrophy, or leprosy.
Tetany
During hypocalcemia, muscles in the hands and feet may experience tonic spasms. The wrists flex, fingers extend, metacarpophalangeal joints bend, the thumb adducts toward the palm, and opposes the little finger.Tightly binding the forearm for three to five minutes can induce this response, termed a "positive calcium-deficiency binding test," aiding diagnosis.
Palmar Erythema
Capillary dilation and congestion causing redness of the palms, particularly prominent on fingertips, termed "palmar erythema." This results from impaired liver function failing to metabolize estrogen. Common in conditions like cirrhosis and hepatocellular carcinoma.
Phalangeal Deformities
In rheumatoid arthritis, finger joints may develop club-like deformities with limited mobility, accompanied by redness, swelling, and increased pain during flare-ups. Such deformities may also occur in osteoarthritis of the elderly. However, it primarily affects the distal interphalangeal joints, with hard nodules present at the affected sites. Gout, caused by abnormal uric acid metabolism, can result in palpable "tophi" at the joints and joint deformities.
Clubbing
Marked hypertrophy of the distal phalanges, giving them a mallet-like appearance, commonly occurs in chronic pulmonary diseases and congenital heart disease. It is associated with prolonged hypoxia and metabolic disorders in the extremities.
Spoon Finger
Also known as "reverse nails," characterized by concave nail beds with upturned edges and a rough surface resembling a "small spoon." Commonly seen in iron deficiency anemia, occasionally in rheumatic fever.
Nail discoloration
Normal nails are smooth, translucent, and pink: pale nail beds indicate anemia; grayish-white, rough, and dull nails suggest onychomycosis or malnutrition; yellow nails are seen in long-term smokers or jaundiced patients; subungual hemorrhage appears reddish; ischemia and hypoxia, such as in congenital heart disease, cause dark purple nails;Certain medications may cause blue discoloration; old hemorrhages or melanomas may darken nails. Observing capillary changes beneath the nail bed with a magnifying glass reveals microcirculation status, aiding early detection of shock.
Finger Tremors
Traditional Chinese medicine attributes trembling hands and inability to perform fine tasks to: fine tremors commonly seen in hyperthyroidism; moderate tremors associated with emotional agitation, fright, strenuous exercise, excessive alcohol consumption, neurasthenia, or Parkinson's disease in the elderly; coarse tremors primarily observed in chronic alcoholism.
