Breast reduction surgery leverages the advantages that the nipple and areola receive blood supply primarily from the subdermal vascular network andand the strong pigment camouflage around the areola. A double-ring incision centered on the areola is designed. The dermal cap formed between the two rings is used to reshape and secure the remaining glandular tissue. This technique offers excellent cosmetic and reconstructive results for mild to moderate breast hypertrophy and ptosis.
Postoperative scars are concealed within the areola, preserving lactation function. This approach simultaneously addresses various breast pathologies, achieving dual benefits of reduction and therapeutic correction.
Specific techniques for breast reduction surgery:
1) Remove the epidermis between the inner and outer rings. Incise skin and subcutaneous tissue along the outer ring down to the glandular tissue, creating a dermal flap.Perform blunt and sharp dissection of the glandular epithelium toward the margins. Perform wedge resection of hypertrophic or diseased breast tissue in the upper and lower quadrants, taking care not to dissect the posterior space to preserve blood supply to the nipple and areola. 2) Flatten the dermal cap and suture it like a bra to the pectoral fascia or marginal glandular tissue. Suspend the upper glandular margin with 7-0 silk sutures to the pectoral fascia at the level of the second rib, typically securing with three to four stitches.
3) Perform a purse-string suture on the outer ring dermis using 0/4 nylon suture, tightening it to match the inner ring opening. Close the subcutaneous layer with 0/1 silk suture and seal the skin incision with 0/3 silk suture. Absorbable sutures may also be used for closure.
