I. What Constitutes an Aesthetically Pleasing Nose
Rhinoplasty Specialist Li Xinfeng: Based on Chinese ethnic characteristics, an aesthetically pleasing nose should harmonize across five dimensions: height, length, width, nasal tip size, nostril shape, and columella shape.Specifically: - From the side, the nose occupies 1/3 of the face's horizontal width. - From the front, the nose occupies 1/5 of the head's vertical height. - The height of the nasal tip equals 1/2 of the nose's length. - The angle between the forehead and nasal bridge should be 120-130 degrees, intersecting the line connecting the inner corners of both eyes. - The angle between the nose and upper lip should be approximately 90 degrees (see Figure 2).The nasal base ideally forms an equilateral triangle, with the columella length equaling one-third of the triangle's height and matching the nasal tip length. The columella width should correspond to the nostril width. Nostrils should be oval-shaped, symmetrical, and ideally not exceed the width of the medial alar angle (see Figures 1 and 2).
II. Who Requires Rhinoplasty?
Li Xinfeng: Two primary groups undergo rhinoplasty. The first comprises individuals with congenital or acquired nasal deformities affecting aesthetics. Beyond restoring normal appearance, this group seeks functional restoration. The second group consists of individuals dissatisfied with their nasal appearance, seeking surgical correction of nasal defects to achieve greater perfection. Common nasal issues among Chinese individuals include bulbous nose, hump nose, low bridge nose, long nose, saddle nose, short nose, and upturned nose. Correction for these conditions is primarily achieved through rhinoplasty.
III. How is Rhinoplasty Performed?
Li Xinfeng: Rhinoplasty is performed under anesthesia. Simple rhinoplasty requires only local anesthesia, but for comprehensive rhinoplasty, or nasal sculpting, combined anesthesia is recommended. The procedure employs a butterfly incision, typically made inside the nostrils and at the base of the columella. The nasal skin is then elevated from the underlying nasal bone and cartilage. Based on the pre-designed plan, sculpted nasal implants are inserted, and the nasal bone and cartilage are reshaped. If necessary, the nasal tip is refined and the nostril shape adjusted. Finally, the skin is sutured back into place.Rhinoplasty with implants usually takes 1-2 hours. If complex sculpting is required, the procedure may take slightly longer. IV. What is the key to successful rhinoplasty? Li Xinfeng: The outcome of rhinoplasty is closely tied to the surgeon's skill level. Within the industry, simple rhinoplasty is not difficult; the challenge lies in comprehensive nasal reshaping. The most difficult aspect of comprehensive rhinoplasty is the design and sculpting of the nasal tip.The pinnacle of comprehensive rhinoplasty is achieving the optimal position and appearance of the patient's nasal tip.
In fact, the human nose can be divided into four golden triangle zones, each playing a crucial role in achieving an aesthetically pleasing nose. All four golden triangles converge at the nasal tip. Therefore, every rhinoplasty procedure must begin with the nasal tip, using aesthetic data as the foundation to shape the nose as closely as possible to these ideal measurements, striving for perfection.
V. What are the golden triangle zones of the nose? What special significance do they hold for rhinoplasty?
Li Xinfeng: Through studying the facial characteristics of Asians, we discovered four golden triangle zones within the nose. By integrating comprehensive facial design based on the patient's facial structure, features, and overall aura, we can achieve the most perfect nasal shape.
First Triangle Zone: An inverted triangle formed by the inner corners of the eyebrows and the lowest point of the nasal root. This area enhances the depth of the eyes and creates strong facial dimensionality.
Second Triangle Zone: A triangle formed by connecting the lowest point of the nose with the starting points of the nasolabial folds on both sides. This zone encompasses the nasal bridge, serving as a transitional link. The angle between the first and second triangles defines the nasofrontal angle.Upper nasal reshaping often focuses on this angle; appropriate refinement can create a vibrant, spirited appearance.
Third Triangular Zone: The starting points of the nasolabial folds on both sides connect with the nasal tip to form a triangular area. This zone serves as the crowning touch for the entire nose and face. It is generally considered most aesthetically pleasing when the nasal tip is 2mm higher than the nasal bridge. The height from the nasal tip to the line connecting the nasolabial folds is also a key factor influencing nasal length. To elongate the nose, extension procedures within this triangular area are necessary. The third triangle primarily emphasizes harmony with other facial features and enhances the three-dimensional contours of the face. The nasal base typically measures 3-4cm in width (slightly wider in males), with nostrils resembling teardrops. When the nasal base is of standard width, the ratio between the columella and the nasal base follows the golden ratio of 0.618. This fourth triangle primarily enhances one's demeanor, conveying an impression of refinement, intelligence, and elegance.
