Many liver disease patients frequently experience fatigue and weakness. Experts indicate this may be an early symptom of cirrhosis. Read on: What causes persistent fatigue and weakness? 1. Fatigue and weakness: One of the early symptoms of cirrhosis, ranging from mild tiredness to severe exhaustion, corresponding to the severity of liver disease activity.Causes of fatigue include: ① Insufficient caloric intake; ② Impaired intermediate metabolism of carbohydrates, proteins, and fats, leading to inadequate energy production; ③ Reduced cholinesterase in the blood due to liver damage or impaired bile excretion, affecting normal neurological and muscular function; ④ Impaired conversion of lactic acid to glycogen, resulting in excessive lactic acid accumulation after muscular activity.
2. Dull, dark complexion: One-third of cirrhosis patients exhibit a duller, darker complexion on the face and around the eye sockets compared to before illness. This results from liver damage and reduced liver function, which increases melanin production.
3. Diarrhea, abdominal distension, and abdominal pain: Diarrhea and distension often result from intestinal wall edema and malabsorption (primarily of fats), niacin deficiency, and parasitic infections. Abdominal pain typically occurs in the upper abdomen, often paroxysmal, and may present as colicky. Pain can also stem from concomitant peptic ulcers, biliary tract diseases, or intestinal infections.
4. Dull pain in the hepatic region: Common in cirrhosis, affecting approximately 60%–80% of patients. Worsens with exertion and is often accompanied by jaundice and fever. Indicates significant hepatocyte damage and holds prognostic significance.
5. Irregular bleeding: Impaired liver function affects the synthesis of prothrombin and other coagulation factors, while splenomegaly leads to thrombocytopenia. Consequently, bleeding from the gums or nose, purpura or petechiae on the skin and mucous membranes, or hematemesis and melena are common.
6. Endocrine dysfunction: Reduced secretion of gonadotropins in cirrhosis can cause testicular atrophy in males. Decreased androgen secretion may lead to gynecomastia and sparse pubic hair in males, while female patients may experience scanty menstruation, amenorrhea, and infertility. Excess estrogen dilates peripheral capillaries, resulting in spider angiomas and palmar erythema.The appearance of spider angiomas indicates progressive liver damage.
