Huanglian (Coptis rhizome) is cold in nature and bitter in taste. The proverb "A mute eating Huanglian"—meaning one who endures bitterness without complaint—aptly captures its essence. It is commonly used to treat damp-heat fullness, vomiting with acid regurgitation, diarrhea with jaundice, high fever with mental confusion, excessive heart fire causing restlessness and insomnia, blood heat causing nosebleeds, red eyes and toothache, boils and carbuncles. Topically applied, it also effectively treats eczema, mouth ulcers, and otitis media.
Side Effects of Coptis
Traditional Chinese medicine possesses complex compositions and pharmacological actions, and current technological capabilities cannot precisely delineate all adverse reactions of Coptis. However, one certainty is that its side effects are minimal.
Coptis is intensely bitter and cold in nature. Overuse or prolonged use may damage the spleen and stomach, causing diarrhea and impairing gastrointestinal transport and transformation functions. This can subsequently affect appetite and lead to indigestion. Use with caution unless there is actual heat in the body.
Huanglian possesses heat-clearing, toxin-resolving, and anti-inflammatory properties, exhibiting inhibitory effects against pathogens like Escherichia coli. It can be brewed as a tea, but is unsuitable for individuals with spleen-stomach deficiency-cold. Its bitter nature may dry fluids and injure body fluids; those with yin deficiency or fluid depletion should use it cautiously due to its bitter-cold nature.
Generally, it should be discontinued once symptoms improve and is not recommended for prolonged use. It is best taken after meals.Dosage: 2-3 grams daily for mild conditions; 5-6 grams daily for severe conditions. Children with underdeveloped spleen and stomach functions should not take Coptis indiscriminately. Methods of Administration Traditional Chinese medicine considers Coptis bitter and cold in nature, excelling at clearing stomach heat and purging stomach fire through its properties of clearing heat, drying dampness, purging fire, and detoxifying.Clinically, Qing Wei San (Stomach-Clearing Powder), a formula primarily featuring Huanglian, is a representative prescription for treating toothache caused by stomach fire. When intense stomach heat transforms into fire, it can lead to discomfort such as swollen or bleeding gums, dry mouth, thirst, craving for cold water or beverages, bitter taste in the mouth, and bad breath. It may also manifest as facial flushing, hard stools, a red tongue, and a yellow tongue coating. When using Coptis to clear stomach fire, it can be taken alone as a powder mixed with water or decocted with other herbs.
Individuals with intense stomach heat may take the Chinese patent medicine Qing Wei Huang Lian Wan (Coptis Stomach-Clearing Pills) or decoct 3 grams of Coptis in water for oral administration. A superior method involves grinding Coptis into a fine powder for later use. Take 50 grams of rice or millet, cook with sufficient water until tender, then place 1.5 grams of Coptis powder into a cup. Add approximately 100 milliliters of the cooked rice water, cover, and steep for 3 minutes.Drink on an empty stomach, once in the morning and once in the evening. Brewing the powder with rice water is preferable to direct decoction with water, as the rice water protects stomach qi and prevents the bitter cold nature of Coptis from harming the stomach.
Individuals with chronic stomach heat may experience liver qi stagnation triggered by negative emotions, which further damages the spleen and stomach, manifesting as acid reflux, stomach discomfort, and nausea. Take 5 grams of Coptis,3 grams of Evodia fruit, and 10 grams of Bupleurum root. Decoct in water and consume to clear liver and stomach heat. Contraindications for Coptis Use 1. Ben Cao Jing Ji Zhu: Contraindicated with Chrysanthemum, Eupatorium, Scrophularia, and Dictamnus; incompatible with Tussilago. 2. Yao Xing Lun: Contraindicated with white silkworm pupae; avoid pork.
3. Shu Ben Cao: It fears Achyranthes root.
4. Compendium of Materia Medica: Coptis is highly potent and extremely cold. It is used to reduce fire and dry dampness. Once the disease is treated, its use should cease. How could it be taken long-term, allowing its destructive force to persist and undermine the body's vital, harmonious energy?
5.Ben Cao Zheng: Huanglian excels at purging heart and spleen fire in cases of excess. Misapplied in deficiency heat, it inevitably repels yang. Thus Kou Zongying cautioned: "In deficiency with cold, use with utmost caution." Wang Haicang advised: "During summer months with prolonged bloody diarrhea, avoid Huanglian—yin resides internally."
6. Commentary on the Classic of Materia Medica: For patients with blood deficiency and qi weakness, spleen-stomach deficiency, and blood insufficiency manifesting as palpitations, insomnia, restlessness, fever, and thirst; postpartum insomnia with blood-deficient fever; diarrhea with abdominal pain; childhood measles; yang-deficient diarrhea; post-lactation diarrhea;elderly with stomach deficiency-cold causing diarrhea, yin-deficient individuals with morning loose stools (a condition termed kidney diarrhea), true yin deficiency, internal heat with restlessness—all such syndromes strictly forbid its use. Violating this contraindication endangers the patient. It most powerfully purges fire and also enters the Liver. Generally, it can enter all meridians like other guiding herbs, but its entry into the Heart is particularly specialized. It stops vomiting and diarrhea, alleviates acid regurgitation, and effectively quenches thirst. It is remarkably potent for treating red eyes, calms the mind, stops nocturnal emissions, stabilizes manic agitation, eliminates fullness and distension, and reduces swelling in the female genitalia. It effectively treats childhood soil-eating disorders, alleviates summer heat, damp-heat, and stagnant heat.
Friendly reminder: Chinese herbs are not without potential side effects; always follow medical advice when using them.
