With the advent of new high-energy pulsed CO2 laser technology, the indications for laser skin resurfacing have expanded beyond those of previous skin resurfacing and ablation techniques. Let's explore the therapeutic indications for laser skin resurfacing together.
1. Skin wrinkles caused by excessive UV exposure and aging are excellent candidates for laser skin resurfacing.The most common wrinkles of this type are found around the eyes and mouth, though they can also appear on the neck, ears, nose, and chest. Wrinkles caused by muscle movement, such as those between the eyebrows and around the nose and lips, cannot be completely removed by laser resurfacing. However, in practice, we have found that forehead wrinkles can indeed be improved by laser resurfacing, though they are more prone to recurrence than wrinkles not caused by muscle movement.
2. CO₂ laser resurfacing offers superior therapeutic and cosmetic outcomes for acne scars, surgical scars, and traumatic scars compared to other modalities. Generally, depressed and non-hypertrophic scars represent the optimal indications for laser resurfacing.Additionally, for latent hypertrophic scars or keloids, after flattening and softening with other techniques and confirming no recurrence after 6–8 weeks of observation, laser resurfacing can blend the scar with surrounding skin tissue, making its borders less noticeable and enhancing cosmetic results.
3. Epidermal lesions such as seborrheic keratoses, melanotic papular dermatoses, and epidermal nevi yield excellent cosmetic results after CO₂ laser vaporization. Benign skin tumors like syringomas, tuberous sclerosis, pilonidal cysts, sebaceous hyperplasia, and pigmented nevi—though rooted in the dermis—can also be removed without complications using this laser technique.It is also effective for treating hypertrophic rosacea and actinic cheilitis, significantly reducing intraoperative bleeding concerns.
Patients sometimes envision laser surgery as a perfect solution capable of eliminating all imperfections without side effects. Therefore, it is essential to conduct preoperative assessments based on the patient's specific skin condition to realistically evaluate achievable outcomes.For instance, patients over 40 with facial and eyelid skin laxity are better suited for facelifts and blepharoplasty. Perioral and periorbital wrinkles respond more effectively to laser therapy. Dense acne scars typically require multiple sessions to achieve satisfactory clinical results.
