As pregnancy progresses into the later stages, many expectant mothers experience swelling in their legs and feet. Shoes may no longer fit, and pants may feel tighter than before. Regarding this, Zhang Yan, Associate Chief Physician of Obstetrics at Peking University Third Hospital, points out that 40%–60% of pregnant women experience swelling after six months of pregnancy. However, some swelling is normal, while other cases may indicate underlying issues.

Generally, if symptoms lessen or disappear after lying down to rest or overnight sleep, it indicates normal physiological edema. This can manifest as swelling from the feet to the ankles, calves, thighs, and occasionally even the perineum.However, some pregnant women may develop generalized edema. This requires heightened vigilance and prompt medical consultation to rule out abnormalities in liver, kidney, or blood pressure functions, such as preeclampsia, renal insufficiency, or liver disease. Severe edema may necessitate diuretic therapy when required.
When edema occurs, pregnant women should note the following:
1. Consume light, low-sodium foods. Avoid excessively salty items, limiting daily salt intake to 5–6 grams. Especially avoid pickled or preserved vegetables to prevent worsening edema.
2. Ensure adequate protein intake. Pregnant women with edema, especially those with malnutrition-related edema, must consume animal-based foods (meat, poultry, fish, shrimp, eggs, dairy) and legumes daily. These foods provide rich, high-quality protein.
3. Increase intake of vegetables and fruits, especially winter melon and watermelon. These contain abundant potassium and fructose that the body cannot rapidly metabolize. They help reduce fluid retention, promote diuresis and detoxification, and are highly effective in preventing and alleviating pregnancy-related edema.
