Many people struggle with the appearance of a short nose. Can rhinoplasty effectively lengthen a short nose? Will it look unnatural? This is a common concern. Since the extended portion isn't natural tissue, could it be prone to complications?
Can a short nose be lengthened?
Short nose lengthening surgery is possible, but it's not simply about adding length. Precise preoperative assessment of associated conditions and meticulous planning are essential for satisfactory results.First, extensive subcutaneous dissection is performed on the nasal dorsum, alae, and tip to facilitate skin stretching and prepare for lengthening. Appropriately thickening the columellar prominence can also add length. Typically, short nose lengthening employs cartilage grafting for the tip. Common approaches include making incisions along the nasal margin inside the nostrils or within the cartilage itself,to expose the alar cartilage and lateral nasal cartilage. Depending on the situation, the cephalic portion of the alar cartilage or the caudal portion of the lateral nasal cartilage may be excised. Alternatively, auricular cartilage grafting can be employed. Cartilage harvested from behind the ear is sculpted and sutured to the apex of the nasal dome, elevating the nasal tip.
Short Nose Augmentation Techniques:
1. Mildly Short Nose
For patients with mildly shortened nasal tips, tip augmentation requires only a slight elevation of the tip.Typically, patients with mild short noses can resolve the issue by grafting auricular cartilage onto the nasal tip. Since auricular cartilage averages about 1mm thick per sheet, 2-4 sheets are usually sutured together to achieve sufficient thickness.
For patients with thicker nasal tip skin, short nose augmentation requires thicker cartilage grafts during transplantation. Typically, patients with thicker nasal skin need grafts approximately 2-3mm thicker than those with thinner skin.
2. Severe Short Nose:
If patients desire a sharp nasal tip resembling European features, septal extension must be combined with short nose augmentation. This technique involves dissecting between the alar cartilages to expose the lower septal cartilage. Graft cartilage is then sutured onto this lower septal area to extend the nasal tip.
The specific approach for short nose augmentation depends on the individual's nasal anatomy. This procedure employs cartilage grafting to sculpt the nasal tip, typically using cartilage harvested from behind the ear. This method yields a natural-looking result that avoids an artificial appearance.
