Traditional Chinese Medicine holds that the kidneys store essence, serving as the origin of qi and blood and the foundation of reproduction; the spleen governs transportation and transformation, acting as the source of qi and blood generation and the root of acquired constitution. The primary causes of habitual miscarriage stem from internal and external factors leading to qi and blood deficiency, unstable kidney qi, and internal heat damaging the fetus. Treatment principles primarily focus on tonifying deficiencies, with particular emphasis on kidney tonification. The therapeutic approach emphasizes combining preventive treatment before conception with syndrome differentiation and treatment to preserve pregnancy after conception. Attention must be paid not only to the woman's own treatment but also to the examination of the man's sperm quality and quantity.
Danggui Shaoyao San (Angelica and Peony Powder) is a renowned formula by the medical sage Zhang Zhongjing, featured in the Golden Cabinet Essential Prescriptions for treating various abdominal pains in women.Japanese medical experts highly regard this formula, unanimously acknowledging its non-teratogenic efficacy in treating habitual miscarriage while promoting embryonic health and reducing miscarriage incidence. Recent studies highlight the critical importance of preconception prophylactic treatment for this condition. Daily oral administration of 8 grams each of Wuzhi Yanzong Pill and Bazhen Pill significantly enhances luteal function in habitual miscarriage patients with qi and blood deficiency and spleen-kidney deficiency.Serum estradiol (E2), progesterone (P), and luteinizing hormone (LH) levels showed statistically significant improvements of varying degrees after two months of treatment compared to pre-treatment levels.
Wuziyanzong Pills, composed of goji berries, dodder seeds, schisandra berries, raspberry fruits, and plantain seeds, replenish essence, nourish marrow, and strengthen the kidneys. As a pure herbal formulation, it effectively regulates sexual gland dysfunction in both men and women. In gynecology, it particularly regulates menstruation, promotes conception, and prevents threatened and habitual miscarriages.
Bazhen Pill is a renowned formula for simultaneously tonifying qi and blood. It is particularly suitable for patients presenting with symptoms such as fatigue, pale or sallow complexion, palpitations and shortness of breath worsening with activity, pale and scanty menstrual flow, aching and heavy sensation in the lower back, and a white coating with a pale tongue.Analysis of clinically effective formulas for 323 cases of habitual abortion reveals common ingredients including Cuscuta seed (Tu Si Zi), Eucommia bark (Du Zhong), Coptis rhizome (Chuan Duan), Mulberry mistletoe (Sang Ji Sheng), Codonopsis root (Dang Shen), Atractylodes rhizome (Bai Zhu), Donkey-hide gelatin (E Jiao), and Polygonum multiflorum root (Shou Wu). For kidney tonification and fetal protection, Cuscuta seed takes precedence; for qi tonification and spleen strengthening, Codonopsis root is paramount; while blood nourishment is best achieved with Polygonum multiflorum root, Rehmannia root (Shu Di), and Goji berries (Gou Qi Zi).
For habitual miscarriage caused by dampness obstruction and qi stagnation with spleen-kidney deficiency, presenting symptoms include vomiting phlegm, mental restlessness, lower back and abdominal pain, fetal restlessness, slight vaginal bleeding, thin white tongue coating, pale red tongue, and fine slippery pulse. The Fetal Stabilizing Decoction (Angelica sinensis, Citrus aurantium, Magnolia officinalis, Cuscuta chinensis, Schizonepeta tenuifolia,Astragalus, Artemisia argyi, Paeonia lactiflora, etc.) to tonify the kidneys, stabilize the fetus, resolve phlegm, and regulate qi. Patients should maintain emotional stability, consume light meals, and rest in bed.
We developed the An Tai Granules (Cuscuta chinensis, Morinda officinalis, Dipsacus asper, Astragalus membranaceus, Eucommia ulmoides,Ramie Root, Ophiopogon, etc.) to treat 105 cases of habitual abortion due to luteal insufficiency. Symptoms included lower back soreness, fatigue, depression, anxiety, palpitations and shortness of breath after activity, pale tongue with thin pulse, and a high basal body temperature phase lasting less than 11 days. Serum progesterone levels were low. Patients took the Pregnancy Preservation Granules twice daily at 20 grams per dose starting from confirmed pregnancy until the fifth month of gestation. Results: 103 pregnancies progressed to term delivery, with only two spontaneous abortions occurring at 4 months gestation. Analysis revealed these two cases exhibited not only the aforementioned "deficiency patterns" but also signs of blood stasis obstruction: scanty menstrual flow with dark purple blood clots, intermittent stabbing pain in chest and abdomen, pale tongue with ecchymotic spots on the edges, and a fine,涩 pulse. Thus, simple qi and blood tonifying agents proved ineffective.Medical Forest Corrections states: "When uterine stasis preemptively occupies space, the fetus finds no room to grow by the third month. Blood fails to nourish the fetus, leading to miscarriage." For such habitual miscarriages, preconception treatment with stasis-resolving herbs is advisable. Modified Lower Abdominal Stasis-Resolving Decoction (containing fennel, roasted ginger, chuanxiong, cinnamon bark, pollen, pentaphyllum, angelica, mugwort, astragalus, etc.) to tonify qi and transform stasis. Once clinical signs of blood stasis (purple spots on the tongue, stabbing abdominal pain, scanty menstruation) resolve, and systemic and uterine blood circulation along with the internal environment for embryo implantation improve, conception may be attempted without risk of recurrence.
The application of simplified dietary therapy formulas offers a convenient approach for treating habitual miscarriage. Starting from confirmed pregnancy, alternate days should involve decocting 50g ramie root as a water substitute, combined with 30g walnut kernels, 20g lotus seed kernels,100 grams of Chinese yam, and 100 grams of glutinous rice. Consuming this mixture daily for three consecutive months can significantly alleviate symptoms such as fatigue, weakness, lower back pain, and shortness of breath associated with habitual miscarriage, improve physical constitution, and prevent miscarriage. This dietary therapy centers on ramie root, renowned for its sweet, cooling properties that replenish yin and promote blood circulation. Combined with sweet, moisturizing Chinese yam, sweet, cooling lotus seeds, and sweet, warming walnut kernels and glutinous rice, it forms a nourishing formula. This gentle tonic strengthens the spleen, nourishes the kidneys, and stabilizes the fetus, making it particularly suitable for patients with weaker constitutions experiencing recurrent miscarriage.
