Patient Ba, female, 25 years old, first consultation on April 2, 2012, from Zhengzhou. Chief complaint: Pregnant for over 50 days with heavy uterine bleeding and mild abdominal discomfort. Provincial hospital ultrasound revealed:revealed a shadow mass in the right ovary measuring approximately 2.6 cm in diameter. Beta-hCG radioimmunoassay detected human chorionic gonadotropin at 360 mIU/ml, leading to a diagnosis of (ovarian) ectopic pregnancy. After two weeks of inpatient treatment, follow-up tests showed unchanged hCG levels at 360 mIU/ml,and the shadow mass in the right ovary remained 2.6 cm in size.
Traditional Chinese Medicine (TCM) Differentiation and Treatment for Ectopic Pregnancy
1. Pattern Identification: Blood Stasis and Stagnation
Treatment Principle: Promote blood circulation, remove blood stasis, reduce swelling, and relieve pain.
Formula: Salvia miltiorrhiza 15g, Paeonia lactiflora 15g, Prunus persica kernel 9g, Curcuma zedoaria 36g, Curcuma longa 36g.
Administration: Decoct in water, take one dose daily, divided into two oral administrations.
2. Pattern Identification: Blood Stasis with Qi Stagnation
Treatment Principle: Promote blood circulation to remove stasis, regulate qi to relieve pain.
Formula: Diaxia Zhuyu Chang Decoction: 6g Five-Spirit Fat (roasted), 9g Angelica sinensis, 6g Ligusticum chuanxiong, 9g Peach kernel (ground into paste), 6g Moutan bark, 6g Red peony root, 76g Wuyao, 3g Corydalis tuber, 9g Licorice, 4.5g Cyperus rhizome, 9g Safflower, 4.5g Citrus aurantium peel.
Administration: Decoct in water. Reduce dosage for mild pain; increase for severe cases. Discontinue when symptoms resolve.
Traditional Chinese Medicine Remedy for Ectopic Pregnancy
Combine and modify the formulas Peach Kernel Decoction with Rhubarb and Cinnamon Twig, Poria Pill, and Birth-Promoting Decoction: Peach Kernel 12g, Rhubarb 12g,6g Magnesium Sulfate, 12g Cinnamon Twig, 12g Poria, 12g Peony Bark, 12g White Peony Root, 24g Angelica Root, 9g Ligusticum Root, 3g Dried Ginger, 30g Sargassum, 6g Fried Licorice Root.Six doses, one daily. First soak in warm water for 30 minutes, then boil over high heat until boiling. Reduce to low heat and simmer for 30 minutes. For the second decoction, add water, bring to a boil over high heat, then reduce to low heat and simmer for 20 minutes. Combine both decoctions and divide into three doses for morning, noon, and evening.
Peach Kernel Decoction with Rhubarb and Licorice is a foundational formula in the Shanghan Lun for treating stasis-heat syndromes. The formula synergistically achieves the effects of activating blood circulation, resolving stasis, eliminating masses, and dispersing nodules.Shenghua Decoction is the foundational formula for treating cold-stasis patterns in Fu Qingzhu's Gynecology. In this formula, Angelica sinensis nourishes and invigorates blood, Ligusticum chuanxiong activates blood and promotes qi circulation, peach kernel breaks up blood stasis and expels stasis, roasted ginger warms meridians, dispels cold, and alleviates pain, while licorice tonifies qi, directs blood movement, and resolves stasis. The herbs interact synergistically to achieve the effects of warming meridians and removing stasis.
Based on antepartum vaginal bleeding with clots and a涩 pulse, stasis obstruction was diagnosed. Further analysis of slightly dry stools and aa thin yellow coating indicating heat accumulation obstruction, and a bland taste without thirst indicating yang deficiency with cold intermingling. The three formulas synergistically achieve the effects of clearing heat with cold, dispelling cold with warmth, invigorating blood and resolving stasis, and eliminating masses and dispersing nodules.
