How should one schedule an appointment for back pain? Should it be with Internal Medicine, Surgery, or Gynecology? To address this, we should first categorize back pain broadly, then discuss the appropriate appointment type for each category.
Classification and Common Conditions
1) Based on cause, back pain can be categorized as follows:
1. Back pain caused by spinal disorders is common in conditions like rheumatoid spondylitis, osteophytosis, tuberculous spondylitis, spinal trauma, and herniated discs.
2. Back pain caused by paraspinal soft tissue disorders is commonly seen in lumbar muscle strain and myofascial inflammation.
3. Back pain caused by spinal nerve root disorders is commonly seen in spinal cord compression, acute myelitis, and radiculitis.
4. Back pain caused by visceral diseases is commonly seen in nephritis, urinary tract infections, urinary stones, cholecystitis, gallbladder stones, pancreatitis, gastric and duodenal ulcers, prostatitis, endometritis, adnexitis, and pelvic inflammatory disease.2) How to Schedule an Appointment
1. Orthopedics: Patients with a history of trauma (including sprains, contusions, impact injuries, etc.) experiencing sudden severe lumbar pain—so severe they cannot stand or walk—should consider acute lumbar trauma or disc herniation and seek orthopedic care. Those without trauma history but whose lumbar pain is movement-related should also consult orthopedics.
2. Nephrology: Patients without trauma history experiencing non-movement-related lower back pain (often described as aching), accompanied by facial and lower limb edema worsening in the morning, oliguria, and hypertension should see nephrology. Patients with lower back pain accompanied by frequent urination, urgency, or dysuria should also register with nephrology.
3. Urology: Patients with sudden-onset lower back pain radiating down the flank toward the lower abdomen, accompanied by hematuria and oliguria, should consult urology.
4. Gastroenterology: Patients primarily experiencing abdominal pain radiating to the lower back, along with digestive symptoms such as nausea, vomiting, acid reflux, or abdominal distension, should consult gastroenterology.
5. Gynecology: Female patients experiencing lower back soreness with increased vaginal discharge and foul odor should visit the gynecology clinic.
