China is a nation with a high prevalence of liver disease. Among malignant tumors, liver cancer ranks third in incidence. In 2014, China recorded 364,800 new liver cancer cases and 318,800 deaths from the disease.Incidence and mortality rates are higher in rural areas than in urban regions, and higher in western regions than in other parts of the country.
Treatment Approaches for Liver Cancer
Dong Jiahong, Executive Director of Tsinghua Changgung Hospital, explains that current liver cancer treatment in China is divided into local and systemic therapies. Local treatments primarily include surgery, transarterial chemoembolization (TACE), percutaneous radiofrequency ablation (RFA), and radiotherapy. Systemic treatments encompass chemotherapy, targeted therapy, and immunotherapy.Early-stage liver cancer, when diagnosed and treated, can achieve a 5-year survival rate of 50%-70%, whereas advanced-stage liver cancer has a 5-year survival rate of only 10%-19%.
In China, the primary challenge in liver cancer treatment is that most patients also have cirrhosis. Combined with the fact that most are diagnosed at an intermediate or advanced stage, this often precludes surgical options and increases the risk of metastasis and recurrence.Clinically, advanced-stage liver cancer not only carries a poor prognosis but also exhibits high rates of recurrence and metastasis. Approximately 60%-70% of liver cancer patients experience recurrence within five years, significantly compromising their long-term outcomes.
Addressing this high recurrence rate, Chinese researchers have identified underlying mechanisms.
Major Breakthrough: Tumor Immune Microenvironment of Early-Stage Recurrent Liver Cancer Decoded
On December 23, the internationally renowned journal Cell published research findings from the team led by Academician Fan Jia at Fudan University Zhongshan Hospital, in collaboration with the BGI Research Institute.
Researchers performed full-length single-cell transcriptome sequencing on 16,498 cells from 18 patients with primary and early recurrent hepatocellular carcinoma. This sequencing provided deep insights into the tumor immune microenvironment of early recurrent hepatocellular carcinoma, revealing that cancer cells in this stage exhibit stronger immune evasion characteristics.
In simpler terms, cancer cells can evade attacks from the patient's immune cells. This allows cancer cells to suppress dendritic cells' ability to activate CD8+ T cells, effectively planting a time bomb for recurrence.
Researchers collected tumor samples from 12 patients with primary liver cancer and 6 patients with early recurrent liver cancer.Through sequencing and comparison, the researchers discovered that recurrent HCC possesses a unique immune ecosystem. Characteristically, it exhibits a reduced proportion of immune-suppressive Treg cells, while the proportions of DCs and innate CD161+CD8+ T cells increase. These cells demonstrate dysfunctional cytotoxicity and low proliferative capacity. This indicates that the tumor immune escape mechanisms in recurrent HCC differ from those in primary HCC.
Understanding these differences in immune evasion mechanisms and the immune microenvironment provides a clear theoretical basis for future immunotherapy approaches to liver cancer and the continued search for novel treatment modalities for recurrent liver cancer.
How can the recurrence rate be reduced after liver cancer surgery?
Currently, given the high recurrence rate after liver cancer treatment, patients should adopt comprehensive preventive measures.
Dietary recommendations for liver cancer patients during recovery include increasing vegetable and whole grain intake. The American Cancer Society advises consuming ample fruits, vegetables, soy products, and whole grains. Overall, maintain a relatively light diet while boosting high-quality protein intake. Avoid greasy, spicy, and irritating foods; prioritize balanced nutrition and smaller, more frequent meals.
Regarding exercise, liver cancer patients should engage in appropriate physical activity during recovery. Find a suitable aerobic exercise that can be sustained long-term—swimming, jogging, brisk walking, or Tai Chi exercises are all viable options. Consistency is key; aim for about 30 minutes of daily activity.
Beyond daily care, regular follow-up examinations are essential. Patients must strictly adhere to medical advice and attend scheduled check-ups. This facilitates early detection of metastasis, expanding treatment options. Clinically, some liver cancer patients have experienced recurrence twice, yet early detection enables effective treatment each time.
Liver cancer treatment has now entered the era of immunotherapy. Chinese researchers have identified distinct immune escape mechanisms in early recurrent liver cancer, providing a solid theoretical foundation for refining treatment approaches and developing novel strategies in future care.
