As a classic antiplatelet agent, aspirin is commonly used clinically to prevent and treat ischemic heart disease, angina pectoris, myocardial infarction, and cerebral thrombosis, playing a vital role in cardiovascular disease prevention. Some patients require long-term aspirin therapy. What precautions should they take during treatment? Pharmacists at Guangzhou Medical University Third Affiliated Hospital remind patients to remember these six essential medication guidelines.
1. Aspirin can be taken either in the morning or evening.
Many patients question the optimal timing for aspirin intake. Liu Shaozhi, Chief Pharmacist at the Department of Pharmacy, Guangzhou Medical University Third Affiliated Hospital, explains that while there is ongoing debate about morning versus evening dosing, some studies suggest cardiovascular events peak between 6:00 AM and 12:00 PM, leading to recommendations for morning administration. However, other research indicates evening aspirin may aid in blood pressure reduction.In reality, the timing of medication intake need not be rigidly confined to morning or evening; consistent, long-term adherence to the regimen is more crucial.
2. Timing Matters for Different Dosage Forms
Huang Zhaoki, Associate Chief Physician at the Cardiovascular Department of Guangzhou Medical University Third Affiliated Hospital, advises that standard tablets—which dissolve in the stomach—should be taken after meals to minimize gastric mucosal damage. Enteric-coated tablets, however, remain intact in gastric fluid and are typically taken 30 minutes to 1 hour before meals.
3. Take medication at a fixed time daily to avoid missed doses
Many patients occasionally forget to take their medication. Does missing a dose matter? Liu Shaozhi explains that the body produces 10%-15% new platelets daily, necessitating regular aspirin intake to inhibit these newly formed platelets.
An occasional missed dose won't significantly impact anticoagulation, but frequent omissions will inevitably increase thrombosis risk. Therefore, taking medication at a consistent time each day is recommended.If you miss a dose and it's close to the next scheduled time when you remember, skip the missed dose and take the next dose as usual. If there's a significant gap before the next scheduled dose, take the missed dose immediately and continue with the regular schedule.
4. Avoid alcohol during medication
Liu Shaozhi cautions against drinking alcohol while taking aspirin. Concurrent alcohol consumption increases the risk of gastric mucosal and liver damage. Additionally, aspirin inhibits the activity of alcohol dehydrogenase (ADH), an enzyme involved in alcohol metabolism, slowing down alcohol breakdown and heightening the risk of alcohol poisoning.
If alcohol consumption is unavoidable, time it away from aspirin dosing to minimize harm.
5. These individuals should be especially vigilant for adverse reactions.
Experiencing adverse reactions like gastrointestinal bleeding may cause fear and reluctance to continue medication.Huang Zhaoki advises that individuals with a history of gastrointestinal disease, those over 65 years old, those taking high-dose aspirin, those concurrently using anticoagulants or antiplatelet agents, and those with a history of smoking or alcohol consumption should consult a healthcare professional before taking aspirin. Appropriate use of gastric protectants may be necessary to reduce the risk of gastrointestinal adverse reactions.
6. Adjust medication for "aspirin resistance"
When aspirin fails to prevent thrombosis, this condition is medically termed "aspirin resistance."Liu Shaozhi advises that in such cases, reassessment is necessary to determine whether other risk factors (such as blood pressure, blood sugar, and blood lipids) are inadequately controlled, whether smoking has been discontinued, and whether medication is taken regularly. Additionally, rule out the possibility of concurrent use of other medications that may affect aspirin efficacy (e.g., nonsteroidal anti-inflammatory drugs). If all these factors are excluded, the aspirin dosage may be appropriately increased or the medication changed under medical guidance.
