I. Six Methods to Assess Fertility
1. Age
Women: 24–29 years old; Men: 25–35 years old. This period represents peak reproductive capacity for both partners.

2. BMI Value

3. Contraceptive History

As long as a certain regularity is maintained, it should not be regarded as menstrual irregularity.The last menstrual period (LMP) refers to the most recent menstrual cycle prior to the consultation date, calculated from the first day of bleeding. Note that the LMP should reflect a cycle with typical duration and flow; abnormal vaginal bleeding should not be mistaken for menstruation. Such bleeding is generally lighter than menstrual flow, may be shorter or longer in duration, and often lacks the regular pattern of normal menstruation.
5. Sexual Experience
For women, sexual experience has relatively less impact. However, for men, overly frequent sexual activity can also hinder conception—a crucial point for male partners to note.
6. Health Status
Optimal health is essential for conception in both partners. Sufficient physical stamina, energy, and adequate nutrition support fetal development. Women with chronic illnesses, cancer, autoimmune disorders, liver disease, brain disorders, or kidney disease may face greater difficulty conceiving.
This occurs because poor health often disrupts reproductive hormones. Individuals with familial conditions, such as chromosomal abnormalities, may also experience ovulation difficulties. Additionally, modern environmental factors—including prolonged exposure to electromagnetic waves, X-rays, or radiation—can increase risks of fetal abnormalities and miscarriage. Pregnant women should therefore avoid such exposures.

II. Optimal Childbearing Age
1. Optimal Childbearing Age for Women: 25–29 Years Old
Obstetricians and gynecologists in China generally consider 25 to 29 years old to be the optimal childbearing age for women.During this period, female fertility peaks, uterine contractions are strongest, and the risk of difficult labor is lower. If the mother is too young—such as 20 years old or younger—complications like pregnancy-induced hypertension and premature birth are more likely, and incomplete pelvic development may cause difficult labor. Conversely, advanced maternal age, especially over 35, increases the likelihood of egg cell abnormalities, thereby raising the incidence of fetal malformations.Data indicates that the incidence of Down syndrome in infants is 1 in 1,500 for women aged 25–29; 1 in 900 for those aged 30–34; rises to 1 in 300 for ages 35–39; and reaches 1 in 40 for women over 45. Advanced maternal age also frequently correlates with other chromosomal abnormalities and congenital deficiencies in physical and intellectual development.
2. The optimal age for men to father children is 25–35 years old.
Men in this age group are energetic, physically robust, and produce sperm of the highest quality. As age increases, chromosomal disorders originating from the father also rise. Data indicates that approximately one-quarter of the extra chromosome 21 in cases of trisomy 21 (Down syndrome) is inherited from the father.Separate studies indicate that neonatal mortality rates rise with paternal age. Children born to fathers over 40 face double the risk of birth defects. Another dataset shows that children with optimal intellectual and physical development were born to fathers averaging 29 years old.
3. Women over 35 are considered advanced maternal age
The optimal childbearing age for women is before 30, and should not exceed 35 at the latest. Giving birth during this prime period benefits both mother and child. Pregnancy after age 35 is classified as advanced maternal age, entering a higher-risk phase.Due to factors like male development and health, men's optimal childbearing age is later than women's. A gap in marriage timing is scientifically understandable. However, men face significant work pressure, especially after age 30. Alcohol consumption during social engagements and hectic lifestyles can damage or deplete sperm quality. Therefore, marrying and having children earlier, when stress levels are relatively lower, often ensures conception during the optimal period with guaranteed sperm quality.
