If the effect is not significant, a second course may be administered: 500 U of HCG intramuscularly every 5 days for 3 months, with follow-up examinations during the course and upon completion.
For children with hypothalamic dysfunction, direct replacement with gonadotropin-releasing hormones (e.g., luteinizing hormone-releasing hormone, LHRH) yields optimal results. To achieve ideal effects, administration should mimic the physiological pulsatile release pattern of the hypothalamus: incremental doses of 25 ng/kg every 2 hours via intranasal spray or subcutaneous injection.
(2) Gonadal dysfunction: For isolated testicular testosterone deficiency, testosterone replacement therapy is indicated. This may involve topical testosterone cream or intramuscular testosterone injections at 25mg every 3 weeks for a total of 4 doses. Treatment may result in penile and scrotal enlargement, occasional pubic hair growth, and in some cases, accelerated spinal development.
2. Surgical Treatment: For cryptorchidism patients unresponsive to endocrine therapy, perform orchidopexy as early as possible.
