How to Diagnose an Ectopic Pregnancy

Diagnosing ectopic pregnancy is often possible through physical signs alone. If a patient exhibits the following criteria, ectopic pregnancy can generally be confirmed.
1. General condition: Significant intra-abdominal bleeding may present with anemic appearance. In cases of massive hemorrhage, patients may show pallor, rapid and weak pulse, hypotension, and other shock symptoms.Body temperature is usually normal, may be slightly low during shock, or slightly elevated (not exceeding 38°C) when blood is absorbed into the abdomen.
2. Abdominal Examination: Significant tenderness and rebound tenderness are present in the lower abdomen, particularly on the affected side, though abdominal muscle rigidity is mild. With heavy bleeding, percussion may reveal shifting dullness.
3. Pelvic Examination: Small amounts of blood are often present in the vagina. If bleeding recurs and accumulates, the mass may progressively enlarge and harden.

If recurrent bleeding accumulates, the mass may progressively enlarge and harden. 3. Pelvic examination: Small amounts of blood are often present in the vagina. In cases of tubal pregnancy without miscarriage or rupture, an enlarged fallopian tube with mild tenderness may be palpable. In cases of miscarriage or rupture, the vaginal posterior fornix is full and tender, with marked cervical motion tenderness.
When diagnosis is challenging, persistent vaginal bleeding, worsening abdominal pain, enlarging pelvic masses, and gradually declining hemoglobin levels may prompt physicians to employ the following auxiliary tests for confirmation.
1.Progesterone measurement: Serum progesterone levels are typically low in ectopic pregnancies but remain relatively stable between 5 and 10 weeks of gestation. A single measurement holds significant diagnostic value. Although normal and abnormal pregnancy levels overlap, making absolute cutoff values difficult to define, serum progesterone below 10 ng/mL (radioimmunoassay) often indicates an ectopic pregnancy with approximately 90% accuracy.
2. Pregnancy Test: The pregnancy test may be positive.
3. Vaginal Puncture of the Posterior Fornix: Withdrawal of dark red, non-clotting blood indicates the presence of a hemorrhagic abdomen. In cases of old ectopic pregnancy, small clots or non-clotting old blood may be aspirated.Blood may not be aspirated if there is no internal hemorrhage, minimal bleeding, a high hematoma location, or adhesions in the rectouterine pouch. Therefore, a negative puncture does not rule out tubal pregnancy. In cases of significant internal bleeding with shifting dullness on abdominal examination, paracentesis may be performed via the lower abdomen. 4. Ultrasound Diagnosis: B-mode ultrasound is helpful for diagnosing ectopic pregnancy.
Sonographic features of ectopic pregnancy:
1. In ectopic pregnancy, the uterus is enlarged but the uterine cavity is empty, with a hypoechoic area appearing adjacent to the uterus.
2. Embryonic bud and primitive cardiac tube activity are typically detectable by ultrasound only at 7 weeks of amenorrhea.
3. Following tubal pregnancy miscarriage or rupture, the perimetrial hypoechoic area lacks the characteristic sonographic features of tubal pregnancy.
5. Endometrial Pathology: Diagnosis relies on diagnostic curettage to rule out intrauterine miscarriage.
6. Laparoscopy: Enhances diagnostic accuracy for ectopic pregnancy. Contraindicated in cases of massive intra-abdominal hemorrhage or shock.However, endometrial changes in ectopic pregnancy are non-specific. Findings may include decidual tissue, a highly secretory phase with or without an A-S reaction, or a mixture of secretory and proliferative phases. Endometrial changes correlate with the presence and duration of vaginal bleeding. Thus, diagnostic curettage alone has significant limitations in confirming ectopic pregnancy.
8: Other Biochemical Markers: Recent studies combining serum CA125 and β-HCG testing have revealed a tendency for serum CA125 levels to rise as β-HCG levels decrease. This can aid in distinguishing whether an ectopic pregnancy has aborted or if the embryo is dead.
When diagnosing ectopic pregnancy, differential diagnosis should be made with the following conditions:
1. Threatened miscarriage in early pregnancy: Abdominal pain is generally mild, uterine size corresponds to gestational age, vaginal bleeding is scant, and there are no signs of internal hemorrhage. Ultrasound can differentiate.However, it can sometimes be difficult to distinguish from ectopic pregnancy, especially in patients without a clear history of missed periods and with irregular vaginal bleeding. Diagnosis often requires confirmation with β-hCG testing.
3. Ovarian cyst torsion: Patients typically have normal menstruation and no signs of internal bleeding. They often report a history of adnexal mass, with marked tenderness at the cyst stalk. Diagnosis is confirmed through gynecological examination combined with ultrasound.
4. Ruptured endometrioma (chocolate cyst) bleeding: Patients have a history of endometriosis. This typically occurs premenstrually or during menstruation, presenting with severe pain that may be accompanied by significant anal pressure. Diagnosis is confirmed by aspirating chocolate-like fluid via posterior vaginal fornix puncture. If the rupture involves blood vessels, signs of internal bleeding may appear.
5. Acute Pelvic Inflammatory Disease (PID) During acute or subacute inflammation, menstrual cessation is uncommon. Abdominal pain is often accompanied by fever, with elevated white blood cell counts and erythrocyte sedimentation rates. Ultrasound may detect adnexal masses or pelvic effusion. Urinary hCG testing aids diagnosis, particularly as inflammatory symptoms like abdominal pain and fever gradually subside or resolve following anti-inflammatory treatment.
6. Surgical Conditions Acute appendicitis typically presents with distinct radiating pain in the right lower abdomen, often accompanied by fever, nausea, vomiting, and elevated blood cell counts. Ureteral stones cause colicky pain on one side of the lower abdomen, often with flank pain on the same side and hematuria. Diagnosis is confirmed through ultrasound and X-ray examinations.
