(1) Strict pre-medication screening is required, including medical history review, comprehensive physical and gynecological examinations, and laboratory tests such as urine pregnancy test, vaginal cleanliness assessment, trichomoniasis and fungal screening, complete blood count, and blood typing. Ultrasound examination should be performed when necessary. (2) Physicians must thoroughly explain the medication regimen, therapeutic effects, and potential side effects. Medication should only be administered after obtaining informed consent.
(3) Current standard dosage: Mifepristone 150–200 mg, administered as a single dose or divided over 3 days. On Day 3, administer a prostaglandin agent at the hospital: either 1 mg carboprost suppository vaginally or 600 μg misoprostol orally, followed by 6 hours of hospital observation.
(4) Monitoring during the abortion process: During hospitalization, in addition to monitoring blood pressure, pulse, and drug side effects, all urine and stool must be retained in clean bedpans. A designated person will examine and record the presence of the gestational sac, its expulsion time, size, and the amount of bleeding. If active bleeding occurs before or after expulsion of the gestational sac, uterotonic agents may be administered or immediate curettage performed to stop bleeding.
(5) If the gestational sac remains undischarged after 6 hours of observation with minimal bleeding, the patient may return home and follow up as scheduled. Should tissue be expelled at home, it must be brought to the physician for examination. Medical abortion typically requires 3 to 4 days to terminate an unintended pregnancy. The medication regimen generally spans 4 days, taken on an empty stomach: one tablet twice daily (morning and evening) for the first three days, followed by three tablets taken together on the fourth day.Typically, little sensation is felt during the first three days. On the fourth day, after taking the medication, pain is usually more severe, though some individuals may experience minimal discomfort.
Medical abortion not only takes a longer time but also generally requires about two weeks of rest afterward. After the procedure, it is important to monitor vaginal bleeding. If the bleeding exceeds the volume of a normal menstrual period or is accompanied by other discomforting symptoms, seek medical attention promptly to avoid unnecessary health complications.
