What Does Excessive Amniotic Fluid Mean?

Amniotic fluid is an essential component for sustaining fetal life. Both excessive and insufficient amniotic fluid levels indicate potential abnormalities in fetal development.
Amniotic fluid volume increases progressively throughout pregnancy. By the late stages, it typically reaches 1000–1500 ml. During mid-to-late pregnancy, the primary source of amniotic fluid is fetal urine, with volume regulation dependent on the balance between production and absorption.The fetus swallows approximately 500–1000 ml of amniotic fluid every 24 hours. Most of this fluid is transported back to the mother via the fetal blood circulation. Absorption of amniotic fluid through the fetal skin surface is more pronounced during early pregnancy.
After mid-pregnancy, the gradual keratinization of fetal epidermal cells reduces this absorption capacity. Additionally, the amniotic epithelium covering the placenta and umbilical cord absorbs a small portion of the fluid.
Amniotic fluid volume varies significantly among individuals. A volume exceeding 2000 ml is termed polyhydramnios, while a volume below 300 ml is termed oligohydramnios.
Amniotic fluid volume can be assessed via ultrasound, but this method only estimates volume by measuring the fluid surrounding the fetus and cannot provide precise quantification.
Common causes of polyhydramnios:
Gestational diabetes:
If you have gestational diabetes that is poorly controlled, you may develop polyhydramnios. Approximately 10% of pregnant women with diabetes are diagnosed with polyhydramnios, typically in the third trimester.
Twins or multiple pregnancies:
Carrying twins or multiples also increases the risk of polyhydramnios.Amniotic fluid excess is particularly common in cases of twin-to-twin transfusion syndrome, where one fetus has too little amniotic fluid while the other has too much.Causes of swallowing difficulties may include pyloric stenosis, cleft lip, cleft palate, or gastrointestinal obstruction. Neurological disorders like neural tube defects or hydrocephalus can also impair swallowing.
Fetal Anemia:
Even more rarely, polyhydramnios may signal severe anemia in the fetus due to conditions like Rh incompatibility or rubella infection.Both conditions can be treated with intrauterine blood transfusions. If caused by erythema infectiosum, the baby may recover spontaneously without intervention.
Risks of Polyhydramnios for Fetus and Mother
1. Effects on the Baby
Generally, the more severe the polyhydramnios, the higher the perinatal mortality rate.Newborns with pronounced polyhydramnios exhibit poorer outcomes. While ultrasound can detect obvious fetal malformations, meticulous care is essential even for apparently normal perinatal infants, as some anomalies remain undetectable by ultrasound and chromosomal abnormalities occur at higher rates.Complications such as preterm labor, cord prolapse, and placental abruption increase in women with polyhydramnios, adversely affecting perinatal outcomes. Pregnancy-associated diabetes and neonatal polycythemia both contribute to poor perinatal prognosis.
2. Effects on the Mother
Polyhydramnios primarily affects the mother through placental abruption, uterine inertia, and postpartum hemorrhage.Sudden removal of large volumes of amniotic fluid reduces intrauterine pressure. The resulting imbalance in pressure between the fetal and maternal surfaces of the placenta can rupture maternal-side placental vessels, leading to premature placental abruption. The enlarged uterus in polyhydramnios cases often overstretches myometrial cells. When fluid is rapidly reduced, smooth muscle cells cannot contract effectively, causing uterine atony and subsequent postpartum hemorrhage after delivery. Additionally, abnormal fetal presentation in polyhydramnios increases the rate of operative deliveries.
Common Causes of Oligohydramnios
1. Maternal Factors: Inadequate maternal fluid intake, hypovolemia, medication effects, gestational hypertension, etc.
2. Fetal Factors: Premature rupture of membranes in early pregnancy, fetal growth restriction, postmaturity, fetal abnormalities (e.g., urinary tract anomalies), placental insufficiency.
3.Other factors: Medical interventions or idiopathic causes.
Effects of Oligohydramnios on Fetus and Mother
Oligohydramnios is a significant indicator of fetal abnormalities or maternal underlying conditions. Even if no fetal abnormalities are detected at delivery, such newborns exhibit higher rates of periodic morbidity and mortality compared to average infants. Therefore, prompt identification of the underlying cause is essential upon detection of oligohydramnios.
