Accurate classification of eye bags aids in selecting the appropriate surgical method for removal, ensuring optimal post-operative outcomes.
1. Purely Thickened Orbicularis Oculi Muscle Type: Due to genetic factors, lower eyelid bags appear early in life. Characterized by prominent, continuous arcs near the lower eyelid margin without skin laxity, this type commonly affects individuals aged 20-32.
2. Isolated Skin Laxity Type: This involves laxity of the lower eyelid and lateral canthus skin without orbital septum laxity, thus lacking protruding orbital fat. Fine wrinkles appear around the eyes, commonly seen in middle-aged individuals aged 33-45.
3. Mild to moderate lower eyelid bulging primarily results from congenital overdevelopment of orbital fat, commonly seen in young adults aged 23-36.
4.Moderate to severe lower eyelid bulging accompanied by skin laxity primarily involves relaxation of the skin, orbicularis oculi muscle, and orbital septum. This causes gravitational prolapse of orbital fat. In severe cases, laxity of the lateral canthal ligament, tarsal plate eversion, and epibulbar prolapse may occur, often leading to tearing. This is commonly seen in middle-aged and elderly individuals aged 45-68.
5. Skin laxity accompanied by a depression between the lower eyelid margin and the orbital rim is also common in middle-aged and elderly individuals. Beyond skin laxity, this condition involves atrophy of orbital fat and pre-tarsal fat.
