Did you know eyelid twitching could indicate facial spasms? Why do eyelids twitch? Beliefs linking twitching to misfortune are superstitions and should not be taken seriously. Severe twitching may signal underlying facial issues—seek medical attention promptly. Let's explore what doctors say and how to recognize serious cases.
According to Dr. Ren Jie, an expert at Capital Medical University's Sanbo Brain Hospital, the eyelid is composed of two muscles. One is the orbicularis oculi muscle, which encircles the eye like a wheel and closes the eyelid when contracted. The other is the levator palpebrae superioris muscle, which opens the eye when contracted. The continuous contraction and relaxation of these two muscles enable the eye to open and close.
Why does the eyelid twitch? It might be facial muscle spasm.
Normally, our eyes blink constantly, spreading tears across the cornea and conjunctiva to nourish and protect the surface structures of the eyeball—a process we barely notice. However, if the nerves controlling the eyelid muscles become overstimulated, they may cause the muscles to contract repeatedly, leading to continuous eyelid movement. This sensation is often perceived as an eyelid twitch.
Is eyelid twitching serious?
Many people dismiss eyelid twitching as insignificant. Some may experience it for over a year before seeking medical attention, often only when it spreads to the mouth. While facial spasm rarely causes severe complications, frequent twitching in one eye can interfere with activities like reading. Eyelid spasm, however, presents more serious issues.
Why does eyelid twitching occur? It might be facial spasm.
Facial spasm patients experience abnormal nerve impulses due to abnormal compression of the facial nerve, causing irregular contraction and relaxation of the orbicularis oculi muscle. This leads to two common clinical symptoms:
1.Frequent blinking: Stimulation at the root of the facial nerve causes the orbicularis oculi and levator palpebrae superioris muscles—controlled by the postcranial temporal branch—to repeatedly contract and relax, resulting in frequent blinking. Most cases occur unilaterally, though bilateral instances occasionally occur.
2. Persistent eye twitching: When the facial nerve supplying these muscles is compressed by blood vessels, simultaneous excitation of both muscles causes repeated contractions, spasms, or tremors, resulting in involuntary eyelid twitching.
Eyelid twitching is one symptom of facial spasm, clinically characterized by paroxysmal, involuntary twitching of one side of the facial muscles. Most twitching is confined to one side of the face. Since the facial nerve branches into the temporal, zygomatic, buccal, and cervical divisions after exiting the skull, resulting in four primary symptoms: blinking, eye twitching, facial twitching, and mouth corner twitching. The zygomatic branch controls facial muscle movement, while the buccal branch governs mouth muscle movement. When abnormal pressure on the facial nerve generates abnormal nerve impulses, causing irregular contraction and relaxation of these muscles, it triggers the other two symptoms:

Facial twitching primarily results from abnormal impulses in the zygomatic branch of the facial nerve, causing frequent facial spasms.
Corner-of-mouth twitching mainly stems from abnormal impulses in the buccal branch of the facial nerve, leading to frequent twitching or drooping at the mouth's edge. Severe cases may involve neck and shoulder muscles.

Other symptoms: A minority of patients experience mild facial pain during twitching (known as spasm pain). Some may also have headaches, tinnitus on the affected side, or hearing loss. Some patients may develop weakness and atrophy of the facial muscles on the affected side, along with loss of taste sensation in the anterior two-thirds of the tongue, due to prolonged spasms. Patients with facial spasm may exhibit the above symptoms. If you experience any of these, seek prompt medical evaluation to avoid delaying treatment.
