I. Gingivitis
Gingivitis is the most common gum disease. Its primary clinical manifestation is bleeding gums, typically occurring during brushing, chewing hard foods, or sucking.The gums appear bright red or purplish-red, swollen, and soft. Tartar, soft deposits, and plaque may accumulate. Occasionally, the gum margins may show erosion, with a small amount of pus discharged upon pressure. Deepening of the gingival sulcus, swelling of the papillae, or sudden pain from a gingival abscess may also occur.Under certain conditions, it may progress to periodontal disease, leading to tooth loss. II. Causes of Gingivitis 1. Dental Plaque. Dental plaque is the primary cause of gingivitis, representing a non-mineralized bacterial deposit adhering to tooth surfaces.While plaque exists in everyone's mouth, not all individuals develop subgingivitis or periodontitis. Additional direct or indirect factors contribute to gingivitis formation.
2. Tartar. During daily eating and chewing, mechanical stimulation causes plaque on tartar to come into close contact with the gums, triggering inflammation.
3. Food impaction. This is the most common cause of localized periodontal tissue damage. In younger individuals, it results from misaligned teeth, while in older adults, it stems from age-related gum recession. Food particles trapped between gums and teeth after eating, if not promptly and thoroughly removed, can easily lead to gum swelling, gum recession, interproximal caries, and halitosis.
4. Sensitive periods. Hormonal fluctuations during early adolescence, menstruation, or pregnancy alter the periodontal tissues' responsiveness to pathogenic stimuli, exacerbating gingival inflammation. Gingivitis commonly occurs during these three phases.
III. Progression of Gingivitis
Mild chronic simple gingivitis affects the free gingiva and papillae. Severe cases may involve the attached gingiva and anterior regions, particularly the mandibular anterior teeth. Bleeding occurs when biting fruit or brushing teeth. Healthy gums do not bleed even with vigorous brushing.
Normal free gingiva is thin and tightly adherent to the tooth surface. Pathological symptoms of gingivitis include:The gingival margin appears congested, reddened, swollen, and soft. The margin thickens, interproximal papillae become rounded and lose their close contact with the tooth surface, and the gingival sulcus deepens. In severe cases, the attached gingiva may exhibit tissue edema, loss of punctate pattern, and a glossy surface. The gingival margin may show erosion or granulation tissue proliferation, with pus discharge from the gingival pocket.
As gingivitis progresses, extensive capillary proliferation and dilation cause severe hyperemia. Inflammatory cells and tissue fluid infiltrate, leading to gingival hypertrophy that may partially cover the tooth crown. The gums appear deep red or dark red and bleed readily on probing.In prolonged cases, fibrous proliferation may occur, causing raised gingival growths. The gums may appear mildly reddened or near-normal in color, with a firmer texture and reduced bleeding. Pathologically, gingivitis affects only the gingiva without involving other periodontal tissues, so teeth remain stable. Radiographic examination shows no abnormalities in the alveolar bone, periodontal ligament, or cementum.
IV. Gingival Symptoms
1. Gingivitis patients exhibit gingival edema. During inflammation, tissue swelling thickens the gingival margin, causing it to detach from the tooth surface. Gingival papillae become rounded and enlarged. When attached gingiva is edematous, punctate patterns may disappear, leaving a smooth, glossy surface.
2. Gingivitis patients exhibit noticeable color changes in the gums. Healthy gums appear pink, whereas in gingival margin inflammation, the free gingiva and papillae turn deep red or dark red. This results from vascular congestion and proliferation within the gingival connective tissue.
3. Patients with gingivitis often exhibit bleeding on probing. Healthy gums do not bleed during brushing or probing of the gingival crevices. With gingivitis, bleeding occurs with light touch and during probing.Bleeding on probing is a crucial objective indicator for diagnosing gingival inflammation. Treatment Methods for Gingivitis Given the clear etiology of gingivitis—where dental plaque is the causative factor and lesions are confined to the gingiva—removing the cause and eliminating plaque yields significant results.For mild cases, scaling (commonly known as teeth cleaning) is typically performed to thoroughly remove tartar and control plaque. Redness, swelling, and discharge of the gums are treated with topical or oral antimicrobial medications. In cases of gingival hyperplasia, gingivoplasty is required—a procedure to excise excess gum tissue and restore the gum's physiological contour.After the above treatments eliminate inflammation and restore gum shape, maintaining and consolidating results requires diligent and proper daily brushing to thoroughly remove plaque. Untreated gingivitis can progress, invading deeper periodontal tissues and developing into periodontitis.
VI. Preventing Gingivitis
1. Supplement calcium for teeth. Osteoporosis can increase gingivitis risk; consume calcium-rich foods to strengthen bones.
2. Remove tartar. Schedule regular professional cleanings to eliminate tartar and prevent gingivitis.
3. Use a soft-bristled toothbrush.Brushing gums and tongue with a soft-bristled brush benefits gum health.
4. Quit smoking and drinking. Tobacco and alcohol deplete vitamins and minerals essential for oral health. Achieving healthy teeth requires abstaining from both.
5. Rinse after meals. Rinsing after eating helps remove food particles, significantly aiding dental health.
6. Eat raw vegetables. Raw vegetables contain abundant fiber that cleans and stimulates teeth and gums, preventing gingivitis.
7. Rotate toothbrushes. Using two toothbrushes alternately helps prevent bacterial cross-contamination.
