How does fluorosis form?

Fluorosis is actually a manifestation of mild fluorosis poisoning; severe fluorosis can lead to skeletal fluorosis (fluorosis of the bones).
Fluoride: A Double-Edged Sword That Can Protect or Destroy Teeth
Mothers know that toothpaste contains fluoride because moderate amounts help prevent cavities. However, they may not realize that excessive fluoride exposure can cause dental fluorosis, skeletal fluorosis, or even fluorosis poisoning, with dental fluorosis being the most common clinical manifestation.Moreover, fluorosis involves damage to the dentin layer. Once fluorosis develops, it is irreversible. Improper use of fluoride toothpaste not only fails to strengthen teeth but can actually ruin healthy teeth. Generally, infants and toddlers should begin brushing with toothpaste and a toothbrush around 18 months of age. However, children under 3 often cannot control their swallowing reflexes and may easily swallow toothpaste.
So how much fluoride is considered excessive? For example, drinking water with fluoride levels above 1ppm (1mg/L) can cause dental fluorosis, and levels exceeding 3ppm (3mg/L) result in a 100% incidence rate. Conversely, insufficient fluoride in drinking water reduces teeth's resistance to cavities.When drinking water contains 1 ppm fluoride, it provides caries prevention without causing dental fluorosis. Additionally, the occurrence of dental fluorosis is influenced by local temperature, calcium and phosphorus intake, and individual variations. For instance, in warmer regions where water consumption is higher, fluoride intake increases accordingly. Imbalances in vitamins A and D, as well as calcium and phosphorus, can all exacerbate the risks associated with fluoride.
Caution with Fluoride Toothpaste for Young Children
Although fluoride's role in cavity prevention was established over half a century ago and fluoridated toothpaste is commonplace, dental professors emphasize that children under 3 years old should strictly avoid fluoride toothpaste.
Why should young children be cautious with fluoride toothpaste? First, children's toothpaste often contains added flavors like fruit or sweet scents, which can tempt young children to swallow it.Statistics show that over 80% of children under three swallow toothpaste. While the amount of fluoride ingested each time is minimal, daily use twice a day over time can lead to excessive accumulation in the body—a potential risk for fluorosis.Some parents dismiss this concern, reasoning that baby teeth will eventually fall out. However, this overlooks the developmental process of teeth. Excessive fluoride intake erodes the dentin of permanent teeth. Although permanent teeth begin replacing baby teeth around age 6, the crowns of these permanent teeth start developing around age 1. Therefore, excessive fluoride intake directly impacts the development of permanent teeth.
Experts also recommend that children aged 3-6 use fluoride toothpaste only under parental supervision, strictly limiting the amount to no more than a pea-sized portion.Children in this age group can understand instructions and exercise self-control, making them suitable for fluoride toothpaste. However, the professor notes that even adults swallow about one-sixth of the toothpaste used per brushing session, with children swallowing more. Therefore, the amount of fluoride toothpaste used each time must be strictly controlled. The practice of filling the entire toothbrush, as depicted in TV commercials, is absolutely inadvisable.
Prevention and Treatment of Fluorosis
Treatment
For mild fluorosis symptoms in babies, restoration can be achieved through polishing. For more severe cases, veneers may be used. However, baby teeth are extremely fragile. Before proceeding with restoration or veneers, consult a specialist.Note: Medication for infant fluorosis is best administered after primary teeth have hardened sufficiently. Children over 5 years old may undergo enamel polishing or veneer treatments.
Prevention
Mild Hydrochloric Acid Wiping: If very faint white spots appear on the teeth, apply 4% hydrochloric acid to a cotton swab and gently wipe the tooth surface for about 10 minutes each time. This helps remove surface stains. Follow up by applying 75% sodium fluoride glycerol to promote enamel remineralization.
Avoid Fluoride-Exceeding Water Sources: If local water contains excessive fluoride without effective reduction measures, consider relocating to a healthier environment to prevent fluorosis caused by high water fluoride levels.
Apply Pit and Fissure Sealants: Sealants are crucial for preventing fluorosis in children.This involves applying a chemical sealant to the tooth surface to prevent fluorosis. Mothers should schedule regular check-ups to monitor the sealant's condition and promptly repair any wear or loss.
Fluoride Varnish: Fluoride varnish is another primary method for preventing fluorosis in children. Compared to sealants, varnish covers a larger surface area but offers less effective protection.
