What dietary restrictions apply for heart pain? Which foods should be avoided? For instance, strong tea or coffee should not be consumed.Angina pectoris is a clinical syndrome caused by insufficient coronary artery blood supply, resulting in acute, transient myocardial ischemia and hypoxia. It is characterized by paroxysmal, crushing pain in the anterior chest, primarily located behind the sternum. The pain may radiate to the anterior left chest and left upper limb, often occurring during exertion or emotional stress. It typically lasts several minutes and resolves with rest or nitrates.
Manifestations of Angina Pectoris
Typical angina presents as crushing, suffocating, or oppressive pain behind the sternum or in the precordial region. The pain may radiate to the left shoulder, back, and inner left arm, or to the neck and throat.Typically lasting 2–5 minutes, rarely exceeding 15 minutes; pain subsides within 5 minutes after rest or sublingual administration of nitroglycerin preparations or fast-acting rescue pills. Severe cases may present with pallor, cold sweats, and a sense of impending doom. These symptoms are often triggered by physical exertion, bending to lift heavy objects, emotional agitation, overeating, exposure to cold, or intense heat.It lasts longer and is more severe, with no clear link to physical activity or emotional excitement. It often occurs late at night, occasionally during afternoon naps or rest periods. The pain is often intense and unbearable, causing the patient to become restless and get up to walk around. Nitroglycerin may have little effect or only provide temporary relief.
Variant angina presents similarly to postural angina but exhibits distinct electrocardiogram (ECG) changes during episodes. Some patients may experience angina as shortness of breath, chest tightness, abdominal distension, or symptoms mimicking biliary or abdominal pain. This atypical presentation often occurs in elderly individuals with diminished pain sensitivity.
Dietary Restrictions for Angina Pectoris
Fatty Meals
Frequent or prolonged consumption of high-fat foods like fried dough sticks or fatty meats can lead to coronary atherosclerosis, narrowing of coronary artery lumen, myocardial ischemia and hypoxia, thereby triggering or exacerbating angina episodes.
Alcohol consumption
Chronic heavy drinking is a risk factor for coronary heart disease. Ethanol and other components in alcohol enter the bloodstream after consumption, accelerating heart rate, increasing blood pressure, causing coronary spasm, and elevating myocardial oxygen demand—all of which can trigger angina.
Spicy and irritating foods
Spicy foods like chili peppers, ginger, scallions, garlic, and Sichuan peppercorns possess pungent, warm, and drying properties. When absorbed into the bloodstream after consumption, they can accelerate heart rate, exacerbate myocardial ischemia and hypoxia, and trigger episodes in angina patients.>Cholesterol-rich foods
Animal brains, bone marrow, liver, other offal, egg yolks, certain fish (e.g., cuttlefish, squid), shellfish (e.g., clams, cockles, mussels, crab roe), and fish roe are all high in cholesterol. Regular consumption increases plasma cholesterol levels, potentially causing or worsening coronary heart disease.

Strong Tea and Coffee
The high levels of theophylline and caffeine in strong tea and coffee stimulate the central nervous system and cardiovascular system, accelerating heart rate and causing arrhythmia. This increases myocardial oxygen consumption, potentially triggering angina pectoris.
Vasoconstrictors
Adrenergic agents like epinephrine, norepinephrine, alamine, and dopamine constrict blood vessels, potentially causing myocardial ischemia. Since coronary artery disease patients have narrowed vessel lumens and reduced blood flow, cautious use of vasoconstrictors is crucial to prevent further blood flow reduction.
Tonifying Medications
Tonifying agents like ginseng, astragalus, and Shiquandaibu pills may exacerbate angina symptoms and hinder treatment.
