Due to improvements in nutrition, health conditions, and living standards, the onset of puberty has advanced compared to a century ago. For example, in the United States from 1850 to 1950, the age of menarche decreased by two months every decade, but has since stabilized.
Physical growth in both males and females encompasses reaching adult height and weight, skeletal development, enlargement of all organs except shrinking lymph nodes, and peak brain mass during adolescence.
Boys experience a growth spurt around ages 13–15, with peak growth rates expected to reach 10–16 cm annually.Girls' growth spurts typically begin around age 11, with peak growth rates reaching 9 cm per year, and the vast majority completing growth by age 13.5. Due to the longer growth period in boys, they generally finish heavier and taller than girls. By age 18, 99% of growth is complete, with boys potentially gaining another 2 cm afterward, while girls gain less. Adolescent development can occur earlier or later, but all reach similar final heights.
Sexual maturation follows a consistent sequence in both sexes. In males, changes begin with scrotal enlargement and testicular growth, followed by penile lengthening and development of the seminal vesicles and prostate. Pubic hair then emerges. Axillary and facial hair appear approximately two years after pubic hair growth. Rapid weight gain typically occurs one year after testicular growth begins.The average age of first ejaculation (ranging from 12.5 to 14 years in the United States) is influenced by psychological, cultural, and biological factors.
Rapid penile growth occurs one year after first ejaculation. Unilateral or bilateral gynecomastia is common in boys during early adolescence, typically resolving within a year.
For most females, breast budding is the earliest visible sign of sexual maturation, followed by a growth spurt.Shortly thereafter, pubic and axillary hair emerge. Menarche occurs 2 to 2.5 years after breast development begins, by which time growth has peaked and slowed. The Tanner staging system provides detailed descriptions of breast development and pubic hair growth phases. If the normal sequence of sexual changes is disrupted, growth abnormalities may indicate pathological causes.
Delayed Sexual Maturation
In boys, delayed sexual development is indicated by: no testicular enlargement by age 13.5, absence of pubic hair by age 15, or genital growth remaining incomplete 5 years after its onset. In girls, delayed sexual development is indicated by: no breast development by age 13, absence of pubic hair by age 14, or absence of menarche 5 years after breast development begins.In both sexes, short stature may also indicate delayed sexual maturation.
Precocious Puberty
Sexual maturation beginning before age 8 in girls or age 9 in boys. True precocious puberty involves enlargement and maturation of the gonads following activation of the hypothalamic-pituitary axis, development of secondary sexual characteristics, and spermatogenesis or oogenesis.
In contrast, pseudoprecocious puberty involves secondary sexual characteristic development due to elevated circulating androgen or estrogen levels. These hormones may be secreted by the gonads or adrenal glands without activation of the hypothalamic-pituitary axis.In boys, causes of pseudopuberty include gonadotropin-secreting tumors such as hepatoblastoma and rare pineal tumors; testicular tumors; enzyme deficiencies; and testicular toxicosis. In girls, causes include ovarian follicular cysts, adrenal enzyme deficiencies, and rare female adrenal tumors.
Testicular toxicosis is a rare disorder causing gonadotropin-independent, familial precocious puberty in boys. In this syndrome, both gametogenesis and steroid production remain unstimulated by increased gonadotropin secretion.
McCune-Albright syndrome presents with a classic triad of pseudoprecocious puberty, associated multiple fibrous dysplasia, and café-au-lait spots.The hypothalamic-pituitary axis generally remains prepubertal, with ovarian cysts consistent with sexual maturation.
Premature pubic hair refers to the appearance of pubic hair before age 8 in girls and before age 9 in boys; premature adrenal function refers to the appearance of both pubic and axillary hair before age 8 in girls and before age 9 in boys; premature breast development refers to breast growth beginning before age 8 in girls.
These signs may indicate precocious puberty, though isolated cases of pubic hair appearance, adrenal function, or breast development may occur without further progression. When these signs appear without initiating puberty, they are likely benign. All such cases require follow-up, particularly to assess the course of pubertal development.
