This reflects the ulcer's cyclical progression through acute activity, gradual healing, and scar formation.Active phases may last weeks or even months, while remission periods can extend for months or years. The frequency of flare-ups and the duration of active/remission phases vary based on individual differences, ulcer progression, treatment efficacy, and measures to consolidate therapeutic effects.
3. Rhythmicity: The mechanism of ulcer pain primarily involves excessive gastric acid secretion irritating the ulcer surface. Consequently, pain from ulcers in different locations exhibits distinct patterns. Gastric ulcer pain typically occurs half an hour after meals, persists for 1–2 hours, gradually subsides, and recurs after subsequent meals, forming a cyclical pattern.When gastric ulcers are located at the pyloric canal or coexist with duodenal ulcers, the pain rhythm may mirror that of duodenal ulcers. However, certain ulcer types exhibit less pronounced rhythmicity, such as ulcers in the elderly and postbulbar duodenal ulcers.It is important to note that sudden changes in the pain pattern may indicate disease progression or the development of complications.When ulcers penetrate the serosal layer or become perforated, pain may radiate to other body areas such as the chest, left upper abdomen, right upper abdomen, or back.
5. Nature and intensity of pain: Pain tolerance varies among individuals, so there is no objective standard for ulcer pain intensity. Its nature depends on the patient's perception and is commonly described as hunger-like discomfort, dull ache, belching, pressure sensation, burning pain, severe pain, or stabbing pain.
