When a Cesarean Section is Unavoidable

Maternal factors: Abnormalities in the birth canal, such as a narrow or malformed pelvis, or a mismatch between pelvic dimensions and fetal head size; threatened uterine rupture; severe pregnancy complications, such as heart disease, diabetes, or chronic nephritis;Severe preeclampsia; ineffective uterine contractions during labor despite oxytocin administration; severe antepartum hemorrhage (e.g., placenta previa, placental abruption); prolonged labor (exceeding 30 hours); advanced maternal age for primiparas (over 35 years); acute herpes or vaginal sexually transmitted infections in the mother.
For pregnant women with indications for cesarean section, the procedure not only reduces suffering but also prevents life-threatening complications.
However, cesarean section carries numerous adverse effects. First, compared to vaginal delivery, it results in greater blood loss, slower postoperative recovery, and reduced lactation. Second, it may trigger postoperative syndromes affecting the urinary, cardiovascular, and respiratory systems, as well as various pathologies of reproductive organs like the uterus, including poor healing of the uterine incision and endometriosis. Third, it can adversely impact subsequent deliveries.
For the fetus, cesarean section is indeed an effective means to save the life of the infant in critical situations. In contemporary times, advances in surgical and anesthetic techniques, improvements in blood transfusion safety, and the development and application of antibiotics have significantly enhanced the safety profile of cesarean delivery, making it a reliable method to ensure the safe birth of the fetus.
Abnormal fetal positions, such as transverse or breech presentation, especially when the foot enters the pelvis first, or persistent occipital posterior position;Dystocia, where the fetus cannot be delivered vaginally; premature placental abruption or umbilical cord prolapse before fetal delivery; fetal distress or hypoxia unresponsive to treatment; other conditions unsuitable for natural birth.
However, cesarean delivery also carries several disadvantages for newborns.First, studies indicate that infants delivered vaginally have IgG levels comparable to their mothers, whereas newborns from cesarean sections lack IgG in their umbilical cord blood. IgG is the primary immunoglobulin in human serum and the only antibody transferred from mother to newborn via the placenta. The absence of IgG in cesarean-delivered infants inevitably reduces their immune resistance, increasing susceptibility to illness.
Additionally, cesarean-delivered infants are prone to respiratory distress syndrome. While in the womb, amniotic fluid fills the fetal lungs. During vaginal delivery, this fluid is expelled from the respiratory tract through compression. In cesarean delivery, the fetus is extracted within seconds without this compression, causing amniotic fluid to remain trapped in the lungs and airways.This can cause breathing difficulties in newborns and lead to more severe consequences.

1. Increased Risk of Pulmonary Embolism
Women who deliver vaginally can typically walk the day after birth.However, some cesarean mothers fear incision pain and remain immobile for a week postpartum, which is highly dangerous and increases risk of pulmonary embolism.
